Comparative efficacy of the high sacral ligament suspension and the iliopectineal ligament suspension for treating patients with uterine prolapse in childbearing age
Objective:To explore the perioperative indicators, the postoperative quality of life and the complications of the laparoscopic high sacral ligament suspension surgery(HUS)and the laparoscopic iliac pubic ligament suspension surgery(LP)for treating patients with uterine prolapse in childbearing age.Methods:103 patients with uterine pro-lapse in childbearing age who were admitted for treatment were selected as the study subjects from January 2022 to De-cember 2022.These patients were divided into group A(46 cases with the treatment of LP)and group B(57 cases with the treatment of HUS)according to the different surgical treatment methods. The perioperative indicators, the complications rate, and the scores of the pelvic organ prolapse quantification(POP-Q)and quality of life of the patients were compared between two groups. Results:There was no significant differences in the operation time and the hospi-talization stay of the patients between the two groups(P>0.05).The intraoperative blood loss(46.46±6.91ml),the indwelling catheter time(2.51±0.58d)and the anal exhaust time(24.15±3.06h)of the patients in group B were sig-nificantly higher than those(40.38±5.16ml,2.21±0.42d and 22.16±3.14h)of the patients in group A.After opera-tion,the rate of the position of POP-Q index points(Aa,Ba,Ap,Bp,C)returned to the normal range, the scores of pelvic floor impact questionnaire short form 7(PFIQ-7)and the sexual life of quality questionnaire 12(PISQ-12)of the patients in the two groups had decreased significantly. The postoperative PISQ-12 score(9.12±1.84 points)of the pa-tients in group B was significantly lower than that(9.97±1.67 points)of the patients in group A(all P<0.05).The incidence of dyspurunia(0)of the patients in group B was significantly lower than that(10.9%)of the patients in group A(P<0.05),but the total incidence of complications(7.0%)of the patients in group B had no significant dif-ferent from that(13.0%)of the patients in group A(P>0.05).Conclusion:The postoperative indicators of the pa-tients with the treatment of LP are better than those of the patients with the treatment of HUS,but HUS can better improve the postoperative quality of life of the patients.
