家庭医生签约服务对居民健康素养影响的倾向得分匹配研究

摘要
目的 利用倾向得分匹配(Propensity Score Matching,PSM)法评价家庭医生签约服务对居民健康素养的影响.方法 采用系统抽样和立意抽样相结合的方法,于2023年1月对成都市某区的居民进行问卷调查,将签约家庭医生服务的居民作为处理组,未签约的居民作为对照组,基于反事实框架,利用PSM法评估平均干预效应.结果 签约居民健康理念得分(10.978分)与未签约居民(10.659分)比较,差异有统计学意义(t=3.34,P＜0.05);健康行为得分分别为(3.527分)和(3.391分),差异有统计学意义(t=2.89,P＜0.05);糖尿病防治知识认知得分为(2.281分)和(2.203分),差异有统计学意义(t=2.01,P＜0.05).结论 家庭医生签约服务有利于提高居民的健康素养水平;家庭医生团队应从慢性疾病基本知识及其健康危害方面对居民加强健康教育,提升居民慢性病防治的主观能动性.
Abstract
Objective To evaluate the impact of family doctor contract services on residents'health litera-cy.Methods A questionnaire survey was conducted combined with systematic sampling and purposive sampling among resi-dents in a certain district of Chengdu City.Residents who signed up for family doctor services were selected as treatment group,while residents who did not sign up were selected as control group.Based on counterfactual framework,propensity score matching(PSM)was used to evaluate the average intervention effect Results The difference of health concept scores between contracted residents(10.978)and non-contracted residents(10.659)was statistically significant(t=3.34,P＜0.05).The scores of health behavior in contracted residents and non-contracted residents were 3.527 and 3.391,respectively.The difference was statistically significant(t=2.89,P＜0.05).The cognitive scores of diabetes pre-vention and treatment knowledge in contracted residents and non-contracted residents were 2.2805 and 2.203,and the difference was statistically significant(t=2.01,P＜0.05).Conclusion Family doctor contract services are beneficial for improving the health literacy level of residents.The family doctor team should strengthen residents'chronic disease health education in terms of basic knowledge and health hazards of chronic diseases,and enhance residents'subjective initiative in the prevention and treatment of chronic diseases.
