目的 我国焦虑问题日益普遍,影响越来越多的中国居民,本研究旨在描述和分析1990-2019年中国居民焦虑症疾病负担的长期变化趋势.方法 数据来源于2019年全球疾病负担研究,选取发病率、标化发病率及伤残调整生命年(Disability Adjusted of Life Year,DALY)率等指标,并利用Joinpoint回归模型分析长期变化趋势.结果 1990-2019年中国儿童青少年14岁及以下人群焦虑症发病率和DALY率呈显著上升态势,15岁以上人群焦虑症发病率和DALY率呈现出下降趋势,其中发病率下降最快的是20~24岁年龄段,平均年度变化百分比(Aver-age Annual Percentage Change,AAPC)为-1.13%(95%CI:-1.23%~-1.02%,P<0.05),DALY 率下降最快的年龄段是25~29 岁(AAPC=-0.88%,95%CI:-0.94%~-0.81%,P<0.05).1990-2019 年中国居民焦虑症发病率和 DALY 率的 AAPC 分别为-0.19%(95%CI:-0.33%~-0.05%)和-0.29%(95%CI:-0.52%~-0.07%),P均<0.05.其中,同年龄段相比,女性发病率和DALY率的AAPC均高于男性.结论 1990-2019年中国居民焦虑症发病率和DALY率整体呈现下降趋势,女性焦虑症发病率高于男性,14岁及以下人群发病率高于其他年龄段.焦虑症疾病负担依旧严重,中国居民心理健康问题仍需全面关注.
Analysis on the changing trend of disease burden of anxiety disorders among Chinese residents
Objective To describe and analyze long-term trends in disease burden of anxiety disorders in Chinese population from 1990 to 2019.Methods Data were obtained from the Global Burden of Disease Study 2019,indicators in-cluded incidence,standardized incidence,rate of disability adjusted of life year(DALY)and long-term trends were ana-lyzed by using Joinpoint regression models.Results The incidence of anxiety disorders and DALY among Chinese children and adolescents aged under 14 years showed significant increase from 1990 to 2019,and the incidence of anxiety disorders and DALY showed decreasing trend among population aged 15 years and above,with the fastest decrease in prevalence a-mong population aged 20-24 years[average annual percentage change(AAPC)=-1.13%,P<0.05]and the fastest decrease in DALY rates among population aged 25-29 years(AAPC=-0.88%,P<0.05).The mean annual percentage changes of anxiety disorder prevalence and DALY rates in Chinese residents from 1990 to 2019 were-0.19%(95%CI:-0.33%to-0.05%,P<0.05)and-0.29%(95%CI:-0.52%to-0.07%,P<0.05),respectively.Compared to same age group,all AAPC in the incidence of anxiety disorders and DALY in female were higher than that in male.Conclusion The incidence of anxiety disorders and DALYs in China showed overall decreasing trend from 1990 to 2019,with higher incidence of anxiety disorders in women than that in men,and higher incidence in population under 14 years old than in other age groups.The disease burden of anxiety disorders remains serious,and it is need to pay attention as national mental health issues.
