家庭功能在先天性心脏病手术患儿父母家庭抗逆力与养育压力间的中介效应

目的 探讨家庭功能在先天性心脏病(Congenital Heart Disease,CHD)手术患儿父母家庭抗逆力与养育压力间的中介效应.方法 采用一般资料调查表、家庭关怀度指数问卷(Family APGAR Index,APGAR)、家庭坚韧性量表(Family Hardiness Index,FHI)以及父母养育压力量表(Parenting Stress Index-Short Form,PSI-SF),对天津市某三级甲等儿童医院的部分CHD手术患儿父母进行调查.采用t检验、单因素方差分析、Pearson相关和结构方程模型对数据进行分析.结果 CHD手术患儿父母家庭功能得分为(6.92±1.75)分;家庭抗逆力得分为(64.25±14.24)分;父母养育压力得分为(90.83±29.04)分.Pearson相关分析结果显示,养育压力得分与家庭功能得分、家庭抗逆力得分呈显著负相关(P＜0.05).Bootstrap检验结果显示,家庭功能在CHD手术患儿父母家庭抗逆力与养育压力间起部分中介作用,其直接效应为-0.259,中介作用为-0.136,总效应为-0.395,间接效应占总效应量34.43％.结论 家庭功能在CHD手术患儿父母家庭抗逆力与父母养育压力间起到中介作用,故临床医护人员应采取有效措施提升CHD手术患儿父母家庭功能和家庭抗逆力水平,进而降低他们的养育压力水平.
Abstract
Objective To explore the mediating effect of family function on family resilience and parenting stress of parents of children with congenital heart disease(CHD)after surgery.Methods Part of parents of children with CHD sur-gery from a tertiary children's hospital in Tianjin City were investigated by using the General Information Questionnaire,Family APGAR Index(APGAR),Family Hardiness Index(FHI)and Parenting Stress Index-Short Form(PSI-SF).The data were analyzed by t-test,one-way analysis of variance,Pearson correlation and structural equation model.Results The score of family function of parents of children with CHD after surgery was(6.92±1.75),the score of family hardiness was(64.25±14.24),the score of parenting stress was(90.83±29.04).Pearson correlation analysis showed that par-enting stress score was significantly negatively correlated with family function score and family resilience score(P＜0.05).Bootstrap test showed that family function played an intermediary role between family hardiness and parenting stress of chil-dren with CHD after surgery.The direct effect was-0.259,the intermediary effect was-0.136,the total effect was-0.395,and the indirect effect accounted for 34.43％.Conclusion Family function plays an intermediary role between family hardiness and parenting stress of children with CHD after surgery.Therefore,clinical medical staffs should take effec-tive measures to improve family hardiness and family function,and then reduce parenting stress.
