昆明市老年人营养状况及营养不良风险评估分析

目的 开展昆明市老年人营养不良风险评估及了解营养状况.方法 2020年9-12月采用多阶段抽样方法抽取3737名65岁以上老年人,使用WS/T552-2017《老年人营养不良风险评估》开展调查,应用等级Logistic回归模型分析营养不良风险影响因素.结果 1813人(48.51%)初筛总分<12分,需继续评估,营养不良率和营养不良风险率分别为20.74%和56.15%,而营养良好率为23.11%.消瘦(OR=1.435,95%CI:1.034~1.990)、患慢性病>3 种(OR=1.833,95%CI:1.461~2.303)、长期服药种类>3 种(OR=2.167,95%CI:1.747~2.762)、睡眠时间<5 h/d(OR=1.697,95%CI:1.416~2.032)、户外独立活动时间<1 h/d(OR=1.952,95%CI:1.590~2.396)、经济状况差和一般(OR=1.723,95%CI:1.209~2.457 和 OR=1.644,95%CI:1.404~1.925)、进食需要依靠别人和稍有困难(OR=14.126,95%CI:9.994~19.945 和OR=3.065,95%CI:2.319~4.055)、餐次为 2 次/d(OR=1.682,95%CI:1.392~2.030)、摄入奶/豆/鱼/肉/禽/蛋类<3 种/d(OR=1.637,95%CI:1.381~1.941)、烹调油摄入量>25 g/d(OR=1.737,95%CI:1.499~2.012)和蔬菜水果<500 g/d(OR=1.800,95%CI:1.531~2.117)是老年人发生营养不良风险的主要危险因素.结论 昆明市社区65岁以上老年人营养状况不佳,当地社区卫生服务中心(站)或公共营养机构应开展多种形式的营养教育与营养干预,改善老年人营养状况.
Risk assessment on nutrition and malnutrition for elderly adults in Kunming City
Objective To carry out nutrition and malnutrition risk assessment for elderly adults in Kunming City.Methods From September to December 2020,multistage sampling method was adopted,3737 residents aged over 65 years were investigated by the Malnutrition Risk Assessment of the Aged(WS/T552-2017),and a ordinal logistic regression model was used to analyze the influencing factors of the malnutrition risk for elderly adults.Results A total of 1813(48.51%)elderly adults had the score less than 12 and were conducted nutrition assessment further,the percentage of malnutrition and risk of malnutrition risk were 20.74%,56.15%,respectively.The percentage of well-nourish were 23.1%.Thin(OR=1.435,95%CI:1.034-1.990),having more than three chronic diseases(OR=1.833,95%CI:1.461-2.303),long-term medication with more than three kinds of medicines(OR=2.167,95%CI:1.747-2.762),sleeping less than five hours a night(OR=1.697,95%CI:1.416-2.032),taking part in outdoor activities for less than one hour every day(OR=1.952,95%CI:1.590-2.396),having general or poor economic condition(OR=1.723,95%CI:1.209-2.457 or OR=1.644,95%CI:1.404-1.925),eating rely on others or eating disorder(OR=14.126,95%CI:9.994-19.945 or OR=3.065,95%CI:2.319-4.055),having two meals a day(OR=1.682,95%CI:1.392-2.030),insufficient daily intake of milk/soy products/fish/meat/poultry/eggs<3 kinds(OR=1.637,95%CI:1.381-1.941),having more than 25 g edible oil every day(OR=1.737,95%CI:1.499-2.012)and insuf-ficient daily intake of vegetables and fruits(<500 g)(OR=1.800,95%CI:1.531-2.117)were the main risk factors for malnutrition/the risk of malnutrition.Conclusion The nutritional status of the elderly in Kunming community is bad.Local community health service centers(stations)or public nutrition institutions should carry out various forms of nu-trition education and nutrition intervention in order to improve the nutritional status of the elderly.
