Intervention effect of hospital-community joint health education model on the management of patients with type 2 diabetes in community
Objective To evaluate the effect of hospital-community joint health education model on blood glucose,self-management and quality of life among type 2 diabetes patients in community,and provide a reference for health educa-tion intervention on patients with diabetes.Methods Using simple random sampling method,a total of 110 diabetic patients who returned to community hospital for management after treatment in Lishui People's Hospital during June 2021 to July 2022 were selected as the study objects.They were divided into two groups.The control group received routine health education,while the observation group received hospital-community joint health education mode.Blood sugar level,self-management score and quality of life before and after the intervention were compared by using x2test and t test.Results There was no significant difference in blood glucose indicators,self-management scores or quality of life scores between the two groups be-fore intervention(P>0.05).After intervention,the levels of HbAlc[(4.12±1.04)%vs.(5.22±1.05)%],2h PG[(7.56±1.24)mmol/L vs.(8.55±1.27)mmol/L],FPG[(4.12±1.02)mmol/L vs.(5.42±1.14)mmol/L]in the observation group were lower than those in the control group(P<0.05).The total scores of self-management(111.18±7.08)and the SF-36(651.50±11.04)in the observation group were higher than the total scores in the control group[self-management(96.36±6.43),SF-36(570.95±10.36)],and the differences were statistically significant(P<0.05).Conclusion The hospital-community joint health education model could effectively improve the blood glucose level of type 2 diabetes patients in community,enhance the self-management ability of patients,and then improve their quality of life.
