Current status of chronic disease patients'care and management and its influencing factors in primary health-care institutions in Jiangsu Province
Objective To understand current situation of care and management of patients with chronic diseas-esand its influencing factors in primary health care institutions under contract service of family doctors in Jiangsu Province,and to provide theoretical basis for improving the efficiency of essential public health services.Methods A multi-stage stratified random sampling method was employed to survey chronic disease patients from 50 primary healthcare institutions across 13 cities in Jiangsu Province during September to November 2019.A multiple linear regression model was used to ana-lyze primary factors influencing the perception of care and management in primary healthcare institutions.Results A total of 1111 chronic disease patients were included in the study.The total score of Chronic Disease Care Assessment Scale was(49.02±11.28)points.The scores for the dimensions of inquiry,advice,compliance,persistence,and planning were(3.73±1.24),(4.06±1.11),(4.17±1.02),(4.10±1.05),and(4.32±0.96)points,respectively.Multiple linear regression analysis revealed that living in rural areas(B=1.781),having heard but unclear about family doctor con-tracting services(B=-5.791),having preliminary understanding of family doctor contracting services(B=-3.669),not having experienced two-way referral services(B=-2.622),and irregular family doctor visits(B=-10.387)were significant factors influencing the total score of the Chronic Disease Care Assessment Scale in patients(P<0.05).Conclu-sion The care and management of patients with chronic diseases in primary medical and health institutions in Jiangsu Prov-ince is generally good.The main influencing factors are the living area of patients,the degree of familiarity with the service content of family doctors,whether family doctors serve regularly and whether they have enjoyed two-way referral.
