体外冲击波疗法对肩袖修复术后关节僵硬的疗效研究

目的:研究在常规康复治疗基础上联合使用体外冲击波治疗关节镜下肩袖修复术后功能恢复阶段活动受限患者的疗效.方法:选取肩袖修复术后继发肩关节僵硬患者97例,分为常规康复组49例和冲击波组48例,常规康复组接受常规康复治疗,冲击波组接受常规康复及体外冲击波治疗.2组分别于治疗前、治疗4周、8周后采用Constant-Murley肩关节功能评分量表(CMS)、视觉模拟评定表(VAS)和被动关节活动范围(PROM)测量评估肩关节功能.结果:治疗4周、8周后,2组CMS评分及PROM均高于治疗前(P<0.01),2组VAS评分均低于治疗前(P<0.01),冲击波组CMS评分及PROM高于常规康复组(P<0.01),VAS评分低于常规康复组(P<0.01).治疗8周后,2组CMS评分及PROM均高于治疗4周后(P<0.01),2组VAS评分均低于治疗4周后(P<0.01).结论:对于关节镜下肩袖修复术后继发肩关节僵硬患者,在常规康复治疗基础上联合使用体外冲击波治疗能够改善患者的肩关节功能,并在减轻疼痛方面疗效显著.
Efficacy of extracorporeal shock wave therapy for joint stiffness after rotator cuff repair
Objective:To summarize the efficacy of extracorporeal shock wave therapy combined with routine reha-bilitation treatment for limited activity in functional recovery stage after arthroscopic rotator cuff repair.Methods:A total of 97 patients with shoulder joint stiffness after arthroscopic rotator cuff repair were analyzed.Among them,48 patients were treated with routine rehabilitation and extracorporeal shock wave therapy as the shock wave group,and 49 patients were treated with routine rehabilitation only(the routine rehabilitation group).CMS score,VAS score and PROM measurement were used to evaluate shoulder joint function before treatment,4 weeks and 8 weeks after treatment.Results:After 4 and 8 weeks of treatment,the CMS score and PROM were significantly improved(P<0.01)and the VAS score was significantly decreased(P<0.01).In the same period,compared with the rou-tine rehabilitation group,the CMS score and PROM were significantly improved and the VAS score was significantly decreased(P<0.01).Conclusion:For patients with secondary shoulder stiffness after arthroscopic rotator cuff re-pair,combined use of extracorporeal shock wave therapy on the basis of routine rehabilitation treatment can improve the shoulder function of patients,and has a significant effect in relieving pain.
