目的 基于改良卒中Framingham(FSP)量表评估社区高血压病患者10年脑卒中发病风险,探讨脑卒中高风险危险因素特征与颈动脉斑块的关系.方法 本研究采用横断面分层调查,基于分层抽样原则,于2020年2~10月选取苏州高新区某街道辖区7个社区高血压病患者,进行问卷调查,并进行血脂及颈动脉超声检查,依据FSP量表评估未来10年脑卒中发病风险.结果 共纳入高血压病患者2 013例,其中女性1 109例、男性904例;年龄54~85岁,平均年龄(67.19±7.33)岁.根据改良FSP量表评估结果,将高血压病患者按10年脑卒中发病风险分为3组:低风险组1 225例(60.85%)、中风险组635例(31.54%)、高风险组153例(7.60%).3组的性别、年龄、颈动脉斑块组间比较,差异均有统计学意义(均P<0.05).多因素Logistic回归分析显示:男性(P=0.044)、≥65岁(P=0.000)及颈动脉斑块(P=0.033)是高血压病患者未来10年脑卒中高风险的危险因素.结论 应重视高血压病患者的脑卒中风险状况,根据人群特征早期进行筛查,进行针对性干预,以预防或减少脑血管相关疾病的发生.
Abstract
Aim To understand the 10-year stroke risk of hypertensive patients in a community based on the improved Stroke Framingham Scale,and explore the characteristics of high-risk risk factors of stroke and the relationship with carotid plaque.Methods A cross-sectional stratified survey was used.Based on the principle of stratified sampling,a questionnaire survey and blood lipid and carotid artery ultrasound examination were conducted on the residents with hypertension on 7 communities in certain district of Suzhou High-tech Zone from February to October 2020.The risk of stroke in the next 10 years was assessed by using the FSP scale.Results A total of 2013 hypertensive patients were included,including 1109 females and 904 males,aged from 54 to 85 years,with an average age of(67.19±7.33)years.According to the improved FSP,the 10-year stroke risk was determined,with 1225 people at low risk(60.85%),635 people at medium risk(31.54%),and 153 people at high risk(7.60%).Gender,age,and carotid artery plaques showed statistically significant differences among the three different risk groups(P<0.05).Multivariate Logistic regression analysis showed that males(P=0.044),≥65 years old(P=0.000),and carotid artery plaques(P=0.033)are high-risk factors for stroke in hypertensive patients over the next 10 years.Conclusion Attention should be paid to the risk of stroke in hypertensive populations.Early screening and targeted interventions should be conducted based on population characteristics to prevent or reduce the occurrence of cerebrovascular related diseases.
