基底节区血管周围间隙扩大与急性脑梗死继发认知损害的相关性研究

摘要
目的 探讨基底节区血管周围间隙(PVS)扩大对急性脑梗死(ACI)继发认知损害的影响.方法 回顾性分析2016年1月至2020年6月收治446例ACI患者的临床资料,根据有无认知损害分为:认知损害组(312例),为认知正常组(134例).收集两组患者的一般临床、实验室及影像学资料,评价ACI患者发生认知损害的影响因素,同时分析基底节区PVS扩大与认知损害的相关性.结果 两组患者年龄、合并冠心病、同型半胱氨酸(Hcy)、纤维蛋白原(FIB)、基底节区PVS扩大及半卵圆中心PVS扩大的严重程度比较,差异均有统计学意义(均P<0.05);多因素分析结果显示,年龄、Hcy水平及基底节区PVS扩大的严重程度是ACI患者认知损害发生的独立影响因素(均P<0.05);认知损害组基底节区PVS扩大病变严重程度与MoCA量表总分、视觉空间与执行能力评分、注意力评分、语言能力评分及定向能力评分均呈负相关(均P<0.05).结论 ACI患者认知损害的发生与年龄、Hcy水平及基底节区PVS扩大病变的严重程度具有相关性;基底节区PVS扩大可作为ACI患者认知损害的早期预测指标.
Abstract
Aim To investigate the influence of perivascular space(PVS)expansion in basal ganglia on cognitive dysfunction secondary to acute cerebral infarction.Methods The clinical data of 446 patients with acute cerebral infarction(ACI)admitted from January 2016 to June 2020 were retrospectively analyzed.All patients were grouped according to the presence or absence of cognitive impairment,one was cognitive impairment group(312 cases)and another was cognitive normal group(134 cases).The general data,laboratory indexes and imaging data were recorded and the influencing factors of cognitive dysfunction in patients with ACI were evaluated.The correlation between EPVS in basal ganglia and cognitive function was analyzed.Results There were significant differences in age,proportion of coronary heart disease,Hcy,FIB,EPVS in basal ganglia and EPVS in semiovale center between the two groups(P<0.05).Multivariate analysis showed that age,Hcy level and the severity of EPVS in basal ganglia were independent influencing factors of cognitive dysfunction in patients with ACI(P<0.05).The severity of EPVS in basal ganglia of patients in impairment group was negatively correlated with MOCA score,visual space and executive ability score,attention score,language ability score and orientation ability score(P<0.05).Conclusion The occurrence of cognitive impariment in patients with ACI should be closely related to age,Hcy level and the severity of EPVs in basal ganglia and EPVS in basal ganglia can be used as the early predictor of cognitive impairment in patients with ACI.
