摘要

目的:探讨脑胶质瘤患者中7.0T超高场强T2WI的图像质量及对肿瘤细节显示的价值.方法:2022年1-6月我院前瞻性招募23例胶质瘤的受试者,所有受试者于同一天内行7.0T及3.0T T2WI扫描.在图像质量方面,针对图像伪影及灰质边界、尾状核、豆状核和幕下区域清晰度进行评估.在肿瘤细节显示方面,对肿瘤流空血管、内部结构及肿瘤边界进行评估.主观评价由两名放射科医生分别进行评估.测量白质、灰质及病灶实质的信噪比(SNR),灰/白质及白质/病灶实质的对比度噪声比(CNR).采用Wilcoxon符号秩和检验比较主观评分之间的差异性,采用配对样本t检验比较客观指标之间的差异.结果:7.0T T2WI对灰质边界、尾状核及豆状核的显示优于3.0T(P<0.05),对肿瘤流空血管、内部结构的显示优于3.0T(P<0.001).对于伪影及肿瘤边界的显示两者无显著差别(P>0.05),7.0T图像幕下区域的清晰度较3.0T差(P<0.001).7.0T T2WI中白质、灰质及病灶实质的SNR均高于3.0T(P<0.001);7.0T T2WI中灰/白质CNR与3.0T相近(P=0.440),白质/病灶实质的CNR低于3.0T(P=0.028).结论:7.0T T2WI显著提升了图像SNR,能够更加清晰识别大脑正常解剖结构,可以观察到更多的肿瘤内部细节,但7.0T对幕下区域的成像效果较差.

Abstract

Objective:To investigate the image quality and the value of tumor detail display of 7.0T ultra-high field strength T2WI in patients with glioma.Methods:Twenty-three subjects with gliomas were recruited in our hospital from Jan-uary 2022 to June 2022.All of them received 7.0T and 3.0T T2WI on the same day.In terms of image quality,image arti-facts and gray matter boundary,caudate nucleus,lenticular nucleus and infratentorial regions were evaluated.In terms of tu-mor details,flow empty vessels,internal structures and tumor boundary were assessed.Subjective evaluation was performed by two radiologists.SNR of white matter,gray matter,and lesion parenchyma,CNR of gray/white matter and white matter/lesion parenchyma were measured.Wilcoxon signed-rank test was used to evaluate the subjective scores.Pared-samples t test was used to compare the differences between the objective indicators.Results:The display of gray matter boundary,caudate nu-cleus and lenticular nucleus on 7.0T T2WI was better than that on 3.0T(P<0.05),and the display of flow empty vessels and internal structure of tumor was better than that on 3.0T(P<0.001).There was no significant difference in the display of arti-facts and tumor boundary between the two(P>0.05).The clarity of infratentorial region of the 7.0T image was worse than that of the 3.0T image(P<0.001).The SNR of white matter,gray matter and lesion parenchyma on 7.0T T2WI was higher than that on 3.0T(P<0.001).CNR of gray/white matter on 7.0T T2WI was similar to that on 3.0T(P=0.440),and CNR of white matter/le-sion parenchyma was lower than 3.0T(P=0.028).Conclusion:Compared with 3.0T,7.0T T2WI significantly improves image SNR,allowing for clearer identification of normal brain anatomy and more internal tumor details.But the images quality of the infratentorial region is poor on 7.0T.
