摘要
目的:探讨应用国际子宫内膜肿瘤分析(IETA)经阴道常规超声联合三维超声(3D US)检查对子宫内膜癌(EC)肌层浸润深度的诊断价值.方法:收集 2020 年 1 月—2022 年 12 月我院经手术病理证实为EC患者 65 例.对所有患者术前 1 周行经阴道二维及 3D US检查,由 2 名经验丰富的超声医师掌握IETA常规超声内容主观评估肌层浸润深度及应用 3D US断层成像与容积成像客观测量病灶到浆膜层的最小距离,以病理结果为金标准比较分析常规超声及联合 3D US对EC肌层浸润的诊断符合率,采用Kappa检验分析超声与病理诊断的一致性.结果:65 例EC患者中,手术病理证实浅肌层浸润 45 例,深肌层浸润 20 例.应用IETA常规超声诊断EC肌层浸润诊断符合率为 83.1%;IETA常规超声联合 3D US断层成像与容积成像诊断EC肌层浸润诊断符合率为 92.3%.结论:IETA常规超声联合 3D US为临床早期评估EC肌层浸润情况提供较为可靠的影像学依据.
Abstract
Objective:To explore the clinical value of combining IETA conventional transvaginal ultrasound with three-di-mensional(3D)ultrasonography in assessing the depth of myometrial infiltration in endometrial cancer(EC).Methods:Sixty-five patients with EC confirmed through surgical procedures and pathological examination at the People's Hospital Affiliated to Shandong First Medical University between January 2020 and December 2022 were selected.They underwent transvaginal 2D and 3D ultrasonography within one week prior to surgery.Two experienced sonographers proficient in IETA conventional ultra-sound techniques were employed to subjectively assess the depth of myometrial infiltration,while 3D ultrasonography was uti-lized to measure the minimum distance between the lesion and the serosal layer.Considering the pathological findings as the gold standard,we compared the diagnostic concordance rate of EC depth of myometrial infiltration between conventional ultra-sound and combined 3D ultrasonography.The Kappa value was used to test the consistency between ultrasound and pathologi-cal results.Results:Sixty-five patients with EC confirmed by surgical pathology were included in this study,comprising forty-five cases of superficial muscular infiltration and twenty cases of deep muscular infiltration.The diagnostic accuracy of utiliz-ing IETA conventional ultrasound for the identification of myometrial infiltration in EC is 83.1%.The diagnostic accuracy of IETA conventional ultrasound combined with 3D ultrasonography in the diagnosis of EC myometrial infiltration reaches 92.3%.Conclusion:The combination of IETA conventional transvaginal ultrasound and 3D ultrasonography provides a reliable ap-proach for the early clinical assessment of myometrial infiltration in patients with EC.
