Correlation analysis of stress elevated blood glucose ratio and senile stroke-associated pneumonia
Objective To investigate the relationship between stress hyperglycemia(SHR)and stroke-associated pneumonia(SAP)in elderly patients with stroke.Methods From February 2019 to February 2021,48 elderly patients with ischemic stroke complicated with SAP were selected as the SAP group,and 72 stroke patients without SAP were selected as the control group in the Neurology ICU of Henan Provincial People's Hospital.The general information,glycosylated hemoglobin(HbA1c)and fasting blood glucose of the two groups at admission to ICU were recorded,and the stress hyperglycemia(SHR)was calculated.According to the follow-up of patients with 28 days prognosis,divided into good prognosis group(n =27)and poor prognosis group(n =21).Multivari-ate Logistic regression and ROC curve were used to analyze the correlation and diagnostic value of SHR with SAP.Results The ratio of SHR in SAP group was significantly higher than that in control group(P<0.05).The age,FBG,SHR,dysphagia,stroke history,indwelling catheter,disturbance of consciousness,mechanical ventilation and NIHSS score of patients in the good prognosis group were lower than those in the poor prognosis group(P<0.05).Logistic regression analysis showed that SHR,age,dysphagia,atrial fi-brillation and NIHSS score were risk factors for SAP(P<0.05).Mechanical ventilation,FBG,SHR and NIHSS score were risk fac-tors affecting the prognosis of SAP(P<0.05).The ROC AUC of SHR for SAP diagnosis and prognosis were 0.797 and 0.790(95%CI =0.713~0.865 and 95%CI =0.648~0.894),respectively and the diagnostic sensitivity was 83.33%and 85.71%,and the specificity was 69.44%and 66.67%.Conclusion SHR has a certain predictive value for the occurrence and prognosis of SAP in elderly stroke patients.Clinical detection of SHR at admission can be used to evaluate the risk of SAP and monitor the prognosis of patients.
