Practice and analysis of clinical pharmacists'drug reorganization for patients with chronic cardiovascular diseases in pri-mary hospitals
Objective Through the practice of clinical pharmacists'drug reorganization for patients with chronic cardiovascular diseases,this paper analyzes the role of clinical pharmacists'drug reorganization service for patients with chronic cardiovascular dis-eases in drug safety and compliance.Methods The patients with chronic cardiovascular diseases admitted to a primary hospital from January to December,2022 were reorganized,and the continuity and rationality of medication orders within 24hours were reviewed,and the problems existing in the orders and the intervention results were summarized and analyzed.Results A total of 154 patients,aged from 23 to84(65.2±11.5)years,were enrolled in the study;patients with1~10 kinds of diseases,with an average of(4.6±1.9)kinds;the patients took 1~11 kinds of western medicines before admission,with an average of(3.9±2.4)kinds.A total of 41 DRPs were found in drug reorganization service,and the reasons were analyzed.Drug selection accounted for 58.5%,dosage selection accounted for 36.6%,and patient-related accounted for 4.9%.Within 24hours,21.43%of patients needed medical intervention,3.2%needed medical advice,and the success rate of clinical pharmacists'intervention was 95.12%.Conclusion Cardiovascular patients are mostly elderly patients,with many complications and many kinds of medication.Clinical pharmacists carry out drug reor-ganization for patients with chronic cardiovascular diseases,find potential drug-related problems,help doctors optimize treatment plans,and improve the safety and compliance of medication for patients with chronic cardiovascular diseases,which can be carried out as routine work in primary hospitals.
