目的:探讨不同类型粘接剂对牙本质粘接强度及耐久性的效果评价.方法:选取2021年6月-2022年6月上海交通大学医学院附属新华医院口腔科就诊的500例患者因治疗需要拔除完整的无龋人离体第三磨牙(共500颗)为研究对象,采用随机数字表法将其分为观察1组(166颗)、观察2组(167颗)和对照组(167颗).观察1组采用全酸蚀粘接剂,观察2组采用二步法自酸蚀粘接剂,对照组采用一步法自酸蚀粘接剂.统计比较不同类型粘接剂的粘接强度、牙本质微渗漏情况及断裂模式.结果:观察1组、观察2组储存24 h、12个月牙本质微拉伸粘接强度均高于对照组,观察1组、对照组储存12个月牙本质微拉伸粘接强度低于储存24 h(P<0.05);储存24 h、储存12个月时,各组牙本质断裂模式主要为混合断裂,三组不同储存时间牙本质断裂模式相比差异无统计学意义(P>0.05);观察1组、观察2组储存24 h、12个月牙本质微渗漏情况均低于对照组,观察1组、观察2组储存12个月牙本质微渗漏情况低于储存24 h,且观察1组较观察2组低(P<0.05).结论:全酸蚀粘接剂、二步法自酸蚀粘接剂的粘接强度较好,随着时间推移,二步法自酸蚀粘接剂的粘接强度仍较高,全酸蚀粘接剂的耐久性相对较好,操作过程中临床可根据实际情况选择合适的粘接剂.
Evaluation of the Effects of Different Types of Adhesives on the Bonding Strength and Durability of Dentin
Objective Explore the effectiveness evaluation of different types of adhesives on the bonding strength and durability of dentin.Methods From June 2021 to June 2022,500 patients in the Department of Stomatology of the author's hospital were selected,and 500 complete and non-carious third molars were extracted for treatment.Using a random number table method,they were divided into observation group 1(166 particles),observation group 2(167 particles),and control group(167 particles).Observation group 1 used full acid etching adhesive,observation group 2 used two-step self acid etching adhesive,and control group used one-step self acid etching adhesive.Compare the bonding strength,dentin micro leakage,and fracture mode of different types of adhesives.Results The micro tensile bonding strength of dentin stored for 24 hours and 12 months in observation group 1 and observation group 2 was higher than that in control group.The micro tensile bonding strength of dentin stored for 12 months in observation group 1 and control group was lower than that stored for 24 hours(P<0.05).When stored for 24 hours and 12 months,the main mode of dentin fracture in each group was mixed fracture,and there was no significant difference in the mode of dentin fracture among the three groups at different storage times(P>0.05).The microleakage of dentin in observation group 1 and observation group 2 after storage for 24 hours and 12 months was lower than that in the control group.The microleakage of dentin in observation group 1 and observation group 2 after storage for 12 months was lower than that after storage for 24 hours,and observation group 1 was lower than observation group 2(P<0.05).Conclusion The bonding strength of the full acid etching adhesive and the two-step self acid etching adhesive is good.Over time,the bonding strength of the two-step self acid etching adhesive is still relatively high,and the durability of the full acid etching adhesive is relatively good.During the operation,suitable bonding agents can be selected according to the actual situation in clinical practice.
