摘要

目的:探讨无托槽隐形矫治器治疗牙周炎致前牙扇形移位的美学效果以及对炎症因子水平的影响.方法:选取2019年3月-2020年3月笔者医院收治的118例牙周炎致前牙扇形移位患者,根据矫正方式不同分为A组(采用自锁托槽矫治器治疗,53例)和B组(采用无托槽隐形矫治器治疗,65例).比较两组牙周指标、炎症因子水平、美学效果评分及临床疗效.结果:治疗6个月后,两组附着丧失(Attachment loss,AL)、龈沟出血指数(Sulcular bleeding index,SBI)、菌斑指数(Plaque index,PLI)、牙周探诊深度(Probing depth,PD)、牙龈指数(Gingivalindex,GI)及白细胞介素-4(Interleukin 4,IL-4)、白细胞介素-6(Interleukin 6,IL-6)、肿瘤坏死因子-α(Tumor necrosis factor alpha,TNF-α)水平均降低,且B组PLI、GI、SBI、PD、AL以及IL-4、IL-6、TNF-α水平均低于A组(P＜0.05);B组美观度、便捷性、语言功能、咀嚼功能评分均高于A组(P＜0.05);B组临床总有效率高于A组(P＜0.05).结论:无托槽隐形矫治器治疗牙周炎致前牙扇形移位美学效果理想,可改善口腔微环境,减少炎症反应,临床有效率高.

Abstract

Objective To investigate the effect of non bracket invisible appliance on the aesthetic effect and the level of inflammatory factors in patients with fan-shaped displacement of anterior teeth caused by periodontitis.Methods 118 patients with fan-shaped displacement of anterior teeth caused by periodontitis admitted to author's hospital from March 2019 to March 2020 were divided into group A(53 patients with self-locking bracket appliance)and group B(65 patients with non bracket invisible appliance)according to different correction methods.The periodontal indexes,inflammatory factor levels,aesthetic effect scores and clinical efficacy were compared between the two groups.Results After 6 months of treatment,attachment loss(AL)sulcular bleeding index(SBI)plaque index(PLI)probing depth(PD)gingivalindex(GI)and interleukin 4(IL-4),interleukin 6(IL-6),tumor necrosis factor alpha(TNF-α)levels of the two groups were decreased,and the PLI,GI,SBI,PD,AL,IL-4,IL-6 and TNF-α levels of group B were lower than those of group A(P＜0.05).After treatment,the scores of aesthetics,convenience,language function,and chewing function in group B were higher than those in group A(P＜0.05).The total clinical effective rate of group B was higher than that of group A(P＜0.05).Conclusion The bracket-free invisible appliance for the treatment of periodontitis-induced fan-shaped displacement of the anterior teeth can improve the oral microenvironment and reduce inflammatory reaction in the treatment of fan-shaped displacement of anterior teeth caused by periodontitis,with ideal aesthetic effect and high clinical efficiency.
