Analysis of the Influence of the Mandible Guided by the Invisible Appliance on the Facial Aesthetic Structure of the Patients with Bone Class Ⅱ Malocclusion
Objective To investigate the effect of invisible mandibular leading appliance on the facial aesthetic structure of patients with skeletal class Ⅱ malocclusion. Methods A total of 96 patients with skeletal class Ⅱ malocclusion from June 2018 to January 2019 in the author's hospital were selected and divided into invisible group(n=46)and double bite pad group(n=50)according to patients'wishes. The group was treated with invisible mandibular leading appliance, and the double occlusal pad group was treated with double occlusal pad appliance. After treatment, the clinical efficacy, bony changes of maxilla and mandible, soft tissue indexes, deep coverage of anterior teeth, distal molar relationship indexes and facial aesthetic structure were compared between the two groups. Results There was no significant difference in ABO average score, maxillary and mandibular bone changes, and soft tissue indexes between the invisible group and the double-bite pad group after treatment(P>0.05).In the invisible group, the proportions of osseous and dental factors were reduced by 66%and 33%,respectively,and the proportions of osseous and dental factors were improved by 67%and 32%respectively in the molar relationship;in the occlusal pad group, the proportions were 58%and 42%,respectively 67%,33%,the coverage reduction in the invisible group with bony factors was higher than that in the double occlusal pad group, and the invisible group with dental factors was lower than the double occlusal pad group(P<0.05).There was no significant difference among the other groups(P>0.05).After treatment, the thickness and R value of the posterior masseter muscle in the invisible group were higher than those in the double occlusal pad group(P<0.05).Conclusion Invisible mandibular leading appliance and double occlusal pad appliance can effectively promote the growth and development of mandible and improve the relationship between mandible and mandibular in patients with skeletal class Ⅱ malocclusion, but invisible mandibular leading appliance is more in line with the aesthetic standards of facial appearance.
