摘要

目的:对比分析复合树脂多层修复技术与复合树脂直接粘接修复技术在前牙间隙美学修复中的效果.方法:纳入笔者医院2020年6月-2021年6月收治的98例(共104颗患牙)前牙间隙患者作为研究对象,按照随机数表法分为A组和B组,各49例.A组患者接受复合树脂多层修复技术,B组患者接受复合树脂直接粘接修复技术.对比两组修复效果、美学效果和治疗总费用,比较两组修复前和修复30 d后、6个月后的咬合力、咀嚼效率和焦虑自评量表(Self-rating anxiety scale,SAS)、抑郁自评量表(Self-rating depression scale,SDS)评分.结果:修复30 d后,A组优秀率高于B组(P＜0.05),两组良好率和不良率比较无统计学意义(P＞0.05).修复6个月后,A组优秀率和不良率均高于B组(P＜0.05),两组良好率比较无统计学意义(P＞0.05).患者的咬合力在时间、交互作用上差异有统计学意义(P＜0.05),在组间作用上差异无统计学意义(P＞0.05).患者的咀嚼效率在时间、组间、交互作用上差异均有统计学意义(P＜0.05).修复6个月后,两组患者咬合力和咀嚼效率均较修复30 d后和修复前升高,且A组咬合力和咀嚼效率高于B组(P＜0.05).A组修复后的边缘密合性、颜色匹配性、修复体崩裂、继发龋损、形态匹配、修复体脱落和牙龈健康评分均低于B组(P＜0.05).患者的SAS评分和SDS评分在时间、组间、交互作用上有统计学意义(均P＜0.05).修复30 d后和修复6个月后,两组患者SAS和SDS评分均较修复前降低,且A组低于B组(P＜0.05).A组治疗总花费高于B组(P＜0.05).结论:与复合树脂直接粘接修复技术相比,在前牙间隙美学修复中应用复合树脂多层修复技术可取得更佳的修复效果,有助于恢复患者咬合力和咀嚼功能,使患者获得良好美学效果,改善其心理状态.但治疗总花费相对较高,临床治疗需结合患者经济情况选择合适治疗方案.

Abstract

Objective To compare and analyze the effects of composite resin multi-layer restoration and composite resin direct bonding restoration in anterior dental space cosmetic repair.Methods A total of 98 patients(104 teeth)with anterior space between teeth who were admitted to the author's hospital from June 2020 to June 2021 were included in the study.They were divided into group A and group B according to the random number table method,with 49 cases in each group.Patients in group A received multi-layer composite resin repair technology,while patients in group B received direct composite resin bonding repair technology.The repair effect,aesthetics and total treatment cost were compared between the two groups.Bite force,mastication efficiency and self-rating anxiety scale(SAS)and self-rating depression scale(SDS)were compared between the two groups before repair,30d after repair and 6 months after repair.Results After 30 days of repair,the excellent rate of group A was higher than that of group B(P＜0.05),and there was no statistical significant difference between the good rate and the bad rate between the two groups(P＞0.05).After half a year's repair,the excellent and bad rates in group A were higher than those in group B(P＜0.05),but there was no significant difference between the two groups(P＞0.05).There were statistically significant differences in the time and interaction of patients'biting force(P＜0.05),but no statistically significant differences in the application between groups.There were significant differences in mastication efficiency of patients in terms of time,intergroup and interaction(P＜0.05).After six months of repair,the bite force and masticatory efficiency in both groups were higher than those after 30 days of restoration and before restoration,and the bite force and chewing efficiency of group A were higher than those of group B(P＜0.05).The edge compatibility,color matching,fracture of restoration,secondary caries,shape matching,loss of restoration and gingival health scores in group A were lower than those in group B(P＜0.05).There were statistical significance in SAS score and SDS score of patients in terms of time,intergroup and interaction(all P＜0.05).After 30 days of repair and half a year of repair,SAS and SDS scores of patients in both groups were lower than those before repair,and those in group A were lower than those in group B(P＜0.05).The total cost of treatment in group A was higher than that in group B(P＜0.05).Conclusion Compared with the composite resin direct bonding repair technology,the composite resin multi-layer repair technology can achieve better repair effect in the aesthetic repair of anterior dental space,which is helpful to restore the bite force and masticatory function of patients,and make patients obtain good aesthetic effect and improve their psychological state.However,the total cost of treatment is relatively high,and appropriate treatment plan should be selected in combination with the economic situation of patients in clinical treatment.
