Effect of CFS Suspension Combined with Tension-free Shortening of Levator Complex in Correction of Moderate and Severe Ptosis
Objective To observe the correction effect of conjoint fascial sheath(CFS)suspension combined with tension-free shortening correction of levator muscle complex on patients with moderate-to-severe ptosis.Methods 128 patients with moderate-to-severe ptosis(238 eyes)admitted to the hospital from January 2019 to October 2022 were selected and divided into two groups according to different surgical methods,including 66 cases in CFS suspension group(122 eyes,CFS suspension)and 62 cases in combined group(116 eyes,CFS suspension combined with tension-free shortening correction of levator muscle complex).The correction effect,eyelid condition,ocular surface function indicators,occurrence of complications,recurrence rate and aesthetic effect were compared between the two groups.Results The effective correction rate in combined group(98.28%)was significantly higher than that in CFS suspension group(92.32%)(P<0.05).The height of palpebral cleft and the corneal distance of upper palpebral margin in combined group were lower than those in CFS suspension group at 1 and 3 months after surgery(P<0.05).At 1 month after surgery,FL in both groups was higher than that before surgery(P<0.05).At 3 months after surgery,FL in both groups was lower than that at 1 month after surgery(P<0.05),and there was significant difference in FL 1 month after operation between the two groups(P<0.05).There was no significant difference in BUT and SIt between the two groups before and after operation(P>0.05).There was no significant difference in the total complication rate and recurrence rate between the two groups(P>0.05).The aesthetic effect scores(eyelid camber,eyelid height symmetry and double eyelid)were significantly higher in combined group than those in CFS suspension group(P<0.05).Conclusion CFS suspension combined with tension-free shortening correction of levator muscle complex has an exact effect in the treatment of patients with moderate-to-severe ptosis.It can improve the eyelid status of patients,and has little impact on ocular surface function and low incidence rates of complications.
