目的:探究生物陶瓷iRoot BP Plus治疗年轻恒牙再生性牙髓的效果及对牙本质桥形成率、钙化物生成体积的影响.方法:回顾性分析2019年1月-2021年12月张家口市口腔医院收治的60例(共64颗患牙)年轻恒牙慢性根尖周炎患儿的临床资料,根据治疗方式不同进行分组,其中将接受常规三氧化矿物凝聚体(Mineral trioxide aggregate,MTA)支架再生牙髓手术者纳入MTA组(n=29,共31颗患牙),接受生物陶瓷iRoot BP Plus支架再生牙髓手术者纳入iRoot BP Plus组(n=31,共33颗患牙).比较两组临床疗效、牙本质桥形成率、钙化物生成体积及根管改善情况,并随访术后1年患牙的恢复情况.结果:iRoot BP Plus组临床总有效率为93.94%,高于MTA组的74.19%(P<0.05).术后1个月及术后3个月,iRoot BP Plus组牙本质桥形成率与钙化物生成体积均高于MTA组(P<0.05).术后3个月,两组患牙根管长度及根管壁厚度均较术前增加,且iRoot BP Plus组高于MTA组(P<0.05).术后1年,iRoot BP Plus组根尖孔封闭、萌出高度增加及牙色泽正常比例均高于MTA组(P<0.05).结论:生物陶瓷iRoot BP Plus治疗年轻恒牙再生性牙髓的疗效确切,能有效促进牙本质桥与钙化物的形成,且有助于根管改善与患牙恢复,值得临床推广应用.
Effects of Bioceramic iRoot BP Plus in Regenerative Endodontic Treatment of Young Permanent Teeth and the Influence on Dentin Bridge Formation Rate and Calcification Volume
Objective The purpose of this study was to investigate the effects of bioceramic iRoot BP Plus in regenerative endodontic treatment of young permanent teeth and the influence on dentin bridge formation rate and calcification volume.Methods The clinical data of 60 children(64 teeth)with chronic apical periodontitis who were admitted to Zhangjiakou Stomatological Hospital from January 2019 to December 2021 were analyzed retrospectively.Among them,children undergoing regenerative endodontic treatment with conventional mineral trioxide aggregate(mineral trioxide aggregate,MTA)stent were defined as the MTA group(29 children,31 teeth)and those undergoing regenerative endodontic treatment with bioceramic iRoot BP Plus stent were defined as the iRoot BP group(31 children,33 teeth).Clinical efficacy,dentin bridge formation rate,calcification volume and improvement of root canal were compared between the two groups.The recovery of affected teeth in 1 year after operation was observed.Results The total clinical response rate in the iRoot BP Plus group(93.94%)was higher than that in the MTA group(74.19%)(P<0.05).The dentin bridge formation rates and calcification volume in the iRoot BP Plus group at 1 month and 3 months after operation were higher/larger than those in the MTA group(P<0.05).Three months after operation,root canal length and wall thickness of the affected teeth in the two groups increased.Root canal length and wall thickness of the affected teeth in the iRoot BP Plus group were larger than those in the MTA group(P<0.05).The proportions of apical sealing,increased eruption height and normal tooth color in the iRoot BP Plus group at 1 year after operation were higher than those in the MTA group(P<0.05).Conclusion Bioceramic iRoot BP Plus is effective in regenerative endodontic treatment of young permanent teeth.It can effectively promote the formation of dentin bridge and calcification,thus contributing to root canal improvement and tooth recovery.
