基于患者美学需求的专项护理模式对断指再植功能恢复和美观满意度的影响

摘要
目的:探究基于患者美学需求的专项护理模式对断指再植术患者功能恢复和创面美观满意度的影响.方法:选取笔者医院2022年1月-2022年12月收治的125例行断指再植术的患者,按照随机数字表法分为观察组62例和对照组63例.对照组采用常规护理,观察组采用常规护理+基于患者美学需求的专项护理.对比术后4周时指关节功能恢复情况[美国手外科协会总主动活动度(TAM)系统],术后4、12周时患者客观感觉[两点辨别觉(Two-point discrimination,2PD)]、瘢痕状态[温哥华瘢痕量表(Vancouver scar scale,VSS)评分]、心理状态[综合性医院焦虑抑郁量表(Hospital Anxiety and Depression Scale,HAD)],术后3、7 d时疼痛程度[视觉模拟评分法(Visual analogue scales,VAS)],术前及术后4、12周时日常生活能力[Barthel指数评定量表(Barthel index,BI)]及术后4周时美观满意度[身体意象量表(Body image scale,BIS)].结果:术后4周时,观察组指功能恢复有效率为96.77%,高于对照组85.11%(P＜0.05),观察组美观满意度为96.77%,高于对照组82.54%(P＜0.05);术后12周时,两组2PD距离、VSS评分、HAD(a)及HAD(d)评分较术后4周时降低,且观察组低于对照组(P＜0.05);术后7 d时,两组VAS评分较术后3 d时降低,且观察组低于对照组(P＜0.05);术后4、12周时,观察组Barthel指数均高于对照组(P＜0.05).结论:基于患者美学需求的专项护理模式能有效促进断指再植术患者客观感觉及指功能恢复,提高创面美观满意度.
Abstract
Objective To explore the effect of patient aesthetic need-based special nursing model on functional recovery and aesthetic satisfaction in patients undergoing severed finger replantation.Methods 125 patients with severed finger replantation admitted to author's hospital from January 2022 to December 2022 were selected and divided into control group(63 cases)and observation group(62 cases)according to the randomized control method.The control group received routine nursing,and the observation group was given routine nursing+patient aesthetic need-based special nursing.The finger joint function recovery[American Society for Surgery of hand total active movement(TAM)system]at 4 weeks after surgery,objective sensation[two-point discrimination(2PD)],scar status[Vancouver scar scale(VSS)]and psychological status[hospital anxiety and depression scale(HAD)]at 4 weeks and 12 weeks after surgery,pain degree[visual analogue scale(VAS)]at 3 and 7 days after surgery,ability of daily living[Barthel index(BI)]before surgery and at 4 and 12 weeks after surgery and aesthetic satisfaction[body image scale(BIS)]at 4 weeks after surgery were compared.Results At 4 weeks after surgery,the effective rate of finger function recovery with 96.77%in experimental group was higher than that 85.11%in control group,and the aesthetic satisfaction with 96.77%was higher than 82.54%in control group(P＜0.05).The 2PD distance,VAS score,HAD(a)score and HAD(d)score in both groups at 12 weeks after surgery were declined compared with those at 4 weeks after surgery,and the above indicators in observation group were lower than those in control group(P＜0.05).At 7 days after surgery,the VSS scores in both groups were reduced compared with those at 3 days after surgery,and the scores were lower in observation group than those in control group(P＜0.05).At 4 and 12 weeks after operation,the Barthel index of the observation group was higher than that of the control group(P＜0.05).Conclusion Patient aesthetic need-based special nursing model can effectively promote the objective sensation and finger function recovery of patients with severed finger replantation,and enhance the wound aesthetic satisfaction.
