目的:探究牙周炎患者牙周再生性手术后不同时机正畸治疗的临床效果.方法:选取笔者医院2020年1月-2021年9月收治的90例牙周炎患者,分为研究组(45例)及对照组(45例),两组患者均行牙周再生性手术,对照组术后6个月行正畸治疗,研究组术后4周行正畸治疗.记录两组菌斑指数(Plaque index,PLI)、牙周探诊深度(Periodontal probing depth,PPD)、临床附着丧失(Clinical attachment loss,CAL)、探查时出血(Bleeding on probing,BOP)及牙周闭锁率,炎性因子水平[白介素-1β(Interleukin-1β,IL-1β)、干扰素-γ(Interferon-γ,INF-γ)及白介素-17(Interleukin-17,IL-17)]以及并发症发生情况.结果:两组牙周袋闭锁率对比,差异无统计学意义(χ2=0.932,P=0.334).正畸治疗后,两组牙周PPD、PLI、CAL及BOP均较治疗前减少(P<0.05),INF-γ、IL-1β及IL-17下降(P<0.05),两组牙周指标、炎性因子水平及并发症比较,差异无统计学意义(P>0.05).结论:牙周炎患者牙周再生性手术完成4周后行正畸可缩短治疗时间,改善牙周组织状况,减少炎症反应,安全性高.
Clinical Observation of Orthodontic Treatment at Different Time in Patients with Periodontitis after Periodontal Regenerative Surgery
Objective To explore the clinical effect of orthodontic treatment at different time after periodontal regenerative surgery in patients with periodontitis.Methods A total of 90 patients with periodontitis admitted to the author's hospital from January 2020 to September 2021 were divided into study group(45 cases)and control group(45 cases).Patients in both groups received periodontal regenerative surgery,the control group received orthodontic treatment 6 months after surgery,and the study group received orthodontic treatment 4 weeks after surgery.Plaque index(PLI),periodontal probing depth(PPD),clinical attachment loss(CAL),bleeding on probing(BOP),periodontal atretic rate,inflammatory factor levels[interleukin-1β(IL-1β),interferon-γ(INF-γ)and interleukin-17(IL-17)]and the incidence of complications were recorded in the two groups.Results There was no significant difference in periodontal pocket atresia rate between the two groups(χ2=0.932,P=0.334).After orthodontic treatment,the PPD,PLI,CAL and BOP in both groups were decreased compared with those before treatment(P<0.05),and INF-γ,IL-1β and IL-17 were decreased(P<0.05),and there was no statistical significance in periodontal indexes,inflammatory factors and complications between the two groups(P>0.05).Conclusion Early orthodontic reoperation 4 weeks after the completion of periodontal regenerative surgery in patients with periodontitis can shorten the treatment time,improve the periodontal tissue condition,reduce inflammatory reaction,and have high safety.
