摘要
目的:讨论一种浅筋膜折叠减张缝合技术在环乳晕法乳房固定术中的临床效果.方法:选取2019年10月-2020年10月笔者科室诊治的26例中度乳房下垂就医者为研究对象,依据术中切口缝合关闭方法的不同,将其分为传统组(n=13)和改良组(n=13).传统组就医者采用传统交锁荷包缝合法,改良组就医者采用浅筋膜折叠减张技术联合交锁荷包缝合法.统计比较两组术后即刻切口满意度、瘢痕情况[温哥华瘢痕量表(Vancouver scar scale,VSS)]及术后12个月与术后即刻乳晕直径差值.结果:术后即刻,就医者对改良组切口的满意度高于传统组(P＜0.05);术后12个月,改良组VSS评分低于传统组(P＜0.05),改良组乳晕增大程度低于传统组(P＜0.05).结论:本研究提出的浅筋膜折叠减张缝合技术在环乳晕法乳房固定术中应用,可获得更加平整、美观的切口对合的同时,能一定程度上减轻术后瘢痕及乳晕扩大的程度,具有良好的临床效果.
Abstract
Objective To investigate the application and clinical effect of a technique of superficial fascia folding and tensioning in circumareolar mastopexy.Methods Twenty-six patients with moderate breast ptosis diagnosed and treated in the author's department from October 2019 to October 2020 were selected as the research objects.According to the different methods of intraoperative incision suture and closure,they were divided into the traditional group(n=13)and the improved group(n=13).Patients in traditional group were treated with traditional locking purse-string suture,and patients in modified group were treated with superficial fascia folding and tension reduction technique combined with locking purse-string suture.The immediate postoperative incision satisfaction,scar condition[Vancouver scar scale(VSS)],and areola diameter difference between 12 months after operation and immediately after operation were compared between the two groups.Results Immediately after operation,the patients'satisfaction with the incision in the modified group was higher than that in the traditional group(P＜0.05).At 12 months after operation,the VSS score of the modified group was lower than that of the traditional group(P＜0.05),and the enlargement of areola in the modified group was lower than that in the traditional group(P＜0.05).Conclusion The superficial fascia folding and tensioning suture technique proposed in this study can obtain more smooth and beautiful incision matching in circumareolar mastopexy,and reduce postoperative scar and areola enlargement to a certain extent,which has good clinical effect.
