Analysis of Aesthetic Evaluation Characteristics and Evaluation Criteria of Shoulder Imbalance in Adolescent Idiopathic Scoliosis Patients
Objective To explore the aesthetic evaluation characteristics of shoulder imbalance in patients with adolescent idiopathic scoliosis(AIS)and analyze evaluation criteria.Methods 48 adolescent idiopathic scoliosis patients admitted to the author's hospital from February 2020 to April 2023 were included as the study subjects,and preoperative back photos of all patients were collected.At the same time,50 observers were selected to evaluate the shoulder imbalance in these back photos,whether they accepted or not.An independent researcher divided the back photos of AIS patients into three regions:neck,shoulder and axillas,and measured 9 aesthetic variables that may affect the imbalance of the trunk and shoulder.Draw ROC curves to analyze the significance of the above data.Results Among the 48 back photos evaluated by 50 observers,31 cases(64.58% )had a consistency of≥80%,of which 19 cases(61.29% )were evaluated as shoulder imbalance and 12 cases(38.71% )were evaluated as no shoulder imbalance.The ROC curve analysis of 19 patients with shoulder imbalance group showed that the ratios of neck bottom angle,trapezius angle difference,shoulder horizontal angle,axillary fold angle,scapular angle and arm width were all P<0.05,and their truncation values were 7.35°,8.43°,4.68°,7.63°,7.01° and 0.43 respectively.ROC curve analysis of 12 patients with shoulder balance group showed that the ratio of axillary plica angle,scapular angle and arm width was all P<0.05,and their truncation values were 3.98°,3.81°and 0.39 respectively.Conclusion Among the three areas in the patient's dorsal photos of the neck,shoulder,and armpit:the angle difference between the base of the neck,trapezius muscle,shoulder horizontal angle,axillary fold angle,scapular angle,and arm width ratio,six variables can be used as aesthetic characteristic variables to evaluate shoulder imbalance in AIS patients.The three variables of axillary fold angle,scapular angle,and arm width ratio can be used as aesthetic characteristic variables to evaluate shoulder balance in AIS patients.And each region has a specific cutoffvalue to define aesthetic acceptance and non acceptance.
