Effects of ultra-thin free peroneal artery perforator neurocutaneous vascular flap repair in treatment of patients with soft tissue defects of hand skin
Objective:To observe effects of ultra-thin free peroneal artery perforator neurocutaneous vascular flap repair in treatment of patients with soft tissue defects of hand skin.Methods:The clinical data of 80 patients with soft tissue defects of hand skin admitted to the hospital from January 2018 to January 2022 were retrospectively analyzed.According to different treatment methods,they were divided into control group and study group,40 cases in each group.The control group was treated with conventional peroneal artery perforator flap repair,while the observation group was treated with ultra-thin free peroneal artery perforator neurocutaneous vascular flap repair.The pain degree[visual analogue scale(VAS)]scores before and after the surgery,the wrist function(wrist Cooney score),the postoperative inflammatory factors[C-reactive protein(CRP),tumor necrosis factor-α(TNF-α),interleukin-6(IL-6)]levels,the efficacy satisfaction,the sensory function recovery 6 months after the surgery,and the incidence of postoperative complications were compared between the two groups.Results:1,3 and 6 months after the surgery,the VAS scores of the two groups were lower than those before the surgery,those of the study group were lower than those of the control group,and the difference was statistically significant(P<0.05).After the surgery,the Cooney scores of the two groups were higher than those before the surgery,that in the study group was higher than that in the control group,and the difference was statistically significant(P<0.05).1 month after the surgery,the levels of CRP,IL-6 and TNF-α in the two groups were lower than those 1 day after the surgery,those in the study group were lower than those in the control group,and the differences were statistically significant(P<0.05).The efficacy satisfaction of the study group was 97.50%(39/40),which was higher than 80.00%(32/40)of the control group,and the difference was statistically significant(P<0.05).The sensory recovery effect of the study group was better than that of the control group,and the difference was statistically significant(P<0.05).However,there was no significant difference in the incidence of postoperative complications between the two groups(P>0.05).Conclusions:Ultra-thin flap peroneal artery perforator flap free repair can improve the wrist function scores and the sensory function and reduce the pain scores and the inflammatory factor levels in the patients with soft tissue defects of hand skin.Moreover,it is superior to conventional peroneal artery perforator flap repair.
