等速肌力训练联合悬吊运动训练在不完全性脊髓损伤患者中的应用效果

目的:观察等速肌力训练联合悬吊运动训练在不完全性脊髓损伤患者中的应用效果.方法:回顾性分析 2020 年 1 月至2022 年 1 月该院收治的 90 例不完全性脊髓损伤患者的临床资料,按照训练方法不同将其分为对照组 40 例和研究组 50 例.对照组实施悬吊运动训练,研究组在对照组基础上联合等速肌力训练,两组均连续训练 3 个月.比较两组临床疗效,训练前后平衡功能[Berg平衡量表(BBS)]评分、下肢肌群表面肌电水平和自理能力[脊髓独立性评定(SCIM)]评分.结果:研究组治疗总有效率为 95.00%(38/40),高于对照组的 77.50%(31/40),差异有统计学意义(P<0.05);训练后,研究组BBS评分以及自我照顾能力、呼吸和括约肌管理能力、移动能力等SCIM评分均高于对照组,差异有统计学意义(P<0.05);训练后,研究组股四头肌、腓肠肌、股二头肌、胫前肌表面肌电水平均高于对照组,差异有统计学意义(P<0.05).结论:等速肌力训练联合悬吊运动训练应用于不完全性脊髓损伤患者可提高治疗总有效率、平衡功能评分、下肢肌群表面肌电水平和自理能力评分,效果优于单纯悬吊运动训练.
Application effects of isokinetic muscle strength training combined with sling exercise training in patients with incomplete spinal cord injury
Objective:To observe application effects of isokinetic muscle strength training combined with sling exercise training in patients with incomplete spinal cord injury.Methods:The clinical data of 90 patients with incomplete spinal cord injury admitted to this hospital from January 2020 to January 2022 were retrospectively analyzed.According to different training methods,they were divided into control group(40 cases)and study group(50 cases).The control group received sling exercise training,while the study group received isokinetic muscle strength training on the basis of that of the control group.Both groups were trained continuously for 3 months.The clinical efficacy,the balance function[Berg balance scale(BBS)]score,the surface electromyography level of lower limb muscles,and the self-care ability[spinal cord independence measure(SCIM)]score were compared between the two groups before and after the training.Results:The total effective rate of the study group was 95.00%(38/40),which was higher than 77.50%(31/40)of the control group,and the difference was statistically significant(P<0.05).After the training,the BBS score and the SCIM scores of self-care ability,respiratory and sphincter management ability,and mobility ability in the study group were higher than those in the control group,and the differences were statistically significant(P<0.05).Further,after the training,the surface electromyography levels of quadriceps femoris,gastrocnemius,biceps femoris and anterior tibial muscle in the study group were higher than those in the control group,and the differences were statistically significant(P<0.05).Conclusions:The isokinetic muscle strength training combined with the sling exercise training in the patients with incomplete spinal cord injury can improve the total effective rate,the balance function scores,the lower limb muscle group surface electromyography levels and the self-care ability scores.Moreover,it is superior to single sling training.
