Effects of Acetylcysteine combined with bronchoscopy alveolar lavage in treatment of children with severe pneumonia
Objective:To observe effects of Acetylcysteine combined with bronchoscopy alveolar lavage in treatment of children with severe pneumonia. Methods:The clinical data of 180 children with severe pneumonia admitted to this hospital from June 2020 to June 2022 were retrospectively analyzed. They were divided into control group and observation group according to different treatment methods,90 cases in each group. Both groups were given routine treatment. On this basis, the control group was treated with bronchoscopy alveolar lavage, while the observation group was treated with Acetylcysteine aerosol inhalation on the basis of that of the control group. Both groups were treated for 2 weeks. The clinical efficacy, the levels of pulmonary function indexes[tidal volume(VT),ratio of time to peak tidal expiratory flow to total expiratory time(TPTEF/TE),ratio of volume at peak tidal expiratory flow to expiratory tidal volume(VPEF/VE)],the T cell subsets indexes(CD3+,CD4+,CD8+,CD4+/CD8+)levels,the serological indexes[C-reactive protein(CRP),tumor necrosis factor-α(TNF-α),soluble vascular endothelial growth factor receptor-1(sFlt-1),placental growth factor(PLGF)]levels, and the incidence of adverse reactions were compared between the two groups before and after the treatment. Results:The total effective rate of the observation group was 96.67%(87/90),which was higher than the control group of 88.89%(80/90),and the difference was statistically significant(P<0.05).After the treatment, the levels of VT,TPTEF/TE and VPEF/VE in the observation group were higher than those in the control group, and the differences were statistically significant(P<0.05).After the treatment, the levels of CD3+,CD4+ and CD4+/CD8+ in the observation group were higher than those in the control group, the level of CD8+ was lower than that in the control group, and the differences were statistically significant(P<0.05).After the treatment, the levels of CRP,TNF-α,sFlt-1 and PLGF in the observation group were lower than those in the control group, and the differences were statistically significant(P<0.05).However,there was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusions:Acetylcysteine aerosol inhalation combined with bronchoscopic alveolar lavage in the treatment of the children with severe pneumonia can improve the total effective rate of treatment and the levels of lung function indexes, improve the levels of T cell subsets indexes, and reduce the levels of serological indexes. Moreover,it is superior to single bronchoscopy alveolar lavage treatment.
