Effects of laparoscopic transabdominal preperitoneal hernia repair in treatment of elderly patients with incarcerated inguinal hernia
Objective:To observe effects of laparoscopic transabdominal preperitoneal hernia repair in treatment of elderly patients with incarcerated inguinal hernia.Methods:A prospective study was conducted on 121 elderly patients with incarcerated inguinal hernia admitted to this hospital from January 2020 to December 2022.According to the random number table method,they were divided into control group(60 cases)and observation group(61 cases).The control group was treated with tension-free hernia repair,while the observation group was treated with laparoscopic transabdominal preperitoneal hernia repair.The perioperative index levels,the pain degree[visual analogue scale(VAS)]score before and after the surgery,the traumatic stress response index[prostaglandin E2(PGE2),corticotropin releasing hormone(CRH),adrenocorticotropic hormone(ACTH)]levels,and the incidence of complications 1 week after the surgery were compared between the two groups.Results:The operation time of the observation group was longer than that of the control group,the anal exhaust time and the out-of-bed time were shorter than those of the control group,the intraoperative blood loss was less than that of the control group,and the differences were statistically significant(P<0.05).After the surgery,the VAS score of the observation group was lower than that of the control group,and the difference was statistically significant(P<0.05).After the surgery,the levels of PGE2,CRH and ACTH in the two groups were higher than those before the surgery,but those in the observation group were lower than those in the control group,the differences were statistically significant(P<0.05).However,there was no significant difference in the incidence of postoperative complications between the two groups(P>0.05).Conclusions:Laparoscopic transabdominal preperitoneal hernia repair in the treatment of the elderly patients with incarcerated inguinal hernia can improve the levels of perioperative indexes,reduce the VAS scores and the levels of traumatic stress response indexes.Moreover,it is superior to tension-free hernia repair.
