Effects of periodontal flap surgery combined with one-time root canal filling in treatment of patients with dental pulp disease
Objective:To observe effects of periodontal flap surgery combined with one-time root canal filling in treatment of patients with dental pulp disease. Methods:A prospective study was conducted on 130 patients with dental pulp disease admitted to the hospital from February 2020 to January 2023.According to the random number table method, they were divided into study group and control group,65 cases in each group. The control group was treated with one-time root canal filling, while the study group was treated with periodontal flap surgery on the basis of that of the control group. The clinical efficacy, the levels of inflammatory indexes[C-reactive protein(CRP),procalcitonin(PCT),white blood cell count(WBC)],the clinical symptom improvement indexes[bite force, masticatory efficiency, numerical rating scale(NRS)]levels, the periodontal related indexes[sulcus bleeding index(SBI),tooth mobility(TM),gingival index(GI),plaque index(PLI)]levels, and incidence of adverse events were compared between the two groups. Results:The total effective rate of treatment in the study group was 92.31% (60/65),which was higher than 80.00% (52/65)in the control group, and the difference was statistically significant(P<0.05).After the treatment, the levels of PCT,CRP and WBC in the two groups were lower than those before the treatment, those in the study group were lower than the control group, and the differences were statistically significant(P<0.05).The masticatory efficiency and bite force of the two groups were higher than those before the treatment, and that in the study group was higher than that in the control group;the NRS scores of the two groups were lower than those before the treatment, and that in the study group was lower than that in the control group;and the differences were statistically significant(P<0.05).The scores of SBI,TM,GI and PLI in the two groups were lower than those before the treatment, those in the study group were lower than those in the control group, and the differences were statistically significant(P<0.05).Further,the incidence of adverse events in the study group was lower than that in the control group, the difference was statistically significant(P<0.05).Conclusions:Periodontal flap surgery combined with one-time root canal filling in the treatment of the patients with dental pulp disease can improve the total effective rate of treatment, improve the levels of clinical symptom improvement indexes, reduce the levels of periodontal related indexes and the incidence of adverse events. Moreover,it is superior to single one-time root canal filling treatment.
