数字化椅旁CAD/CAM制作高嵌体修复牙体缺损的效果

目的:观察数字化椅旁计算机辅助设计/计算机辅助制作技术(CAD/CAM)制作高嵌体修复牙体缺损的效果.方法:选取2022 年 4 月至 2023 年 2 月该院收治的 80 例牙体缺损患者进行前瞻性研究,按照随机数字表法将其分为对照组和研究组各 40 例.对照组采用常规印模制作高嵌体修复,研究组采用数字化椅旁CAD/CAM制作高嵌体修复,比较两组修复成功率、治疗前后龈沟液炎性因子[中核因子κB受体活化因子(RANK)、巨噬细胞炎性蛋白-1α(MIP-1α)]水平、疼痛程度[数字疼痛评分法(NRS)]评分、牙周指标[牙龈指数(GI)、龈沟出血指数(SBI)、牙齿松动度(TM)、菌斑指数(PLI)]水平、咀嚼效率、咬合力和随访 3 个月内并发症发生率.结果:研究组修复成功率为93.02%(40/43),高于对照组的75.61%(31/41),差异有统计学意义(P<0.05);治疗后,研究组咀嚼效率、咬合力均高于对照组,并发症发生率和龈沟液RANK、MIP-1α水平,以及NRS、SBI、TM、GI、PLI评分均低于对照组,差异有统计学意义(P<0.05).结论:数字化椅旁CAD/CAM制作高嵌体修复牙体缺损能够提高修复成功率,改善咀嚼功能和牙周功能,减轻疼痛和炎症刺激,降低并发症发生率,效果优于常规印模制作高嵌体修复.
Effects of digital chair-side CAD/CAM Onlay on repair of tooth defects
Objective:To observe effects of digital chair-side computer-aided design/computer-aided manufacturing(CAD/CAM)Onlay on repair of tooth defects.Methods:A prospective study was conducted on 80 patients with tooth defects admitted to this hospital from April 2022 to February 2023.They were divided into control group and study group according to the random number table method,40 cases in each group.The control group was treated with conventional impression to make Onlay repair,while the study group was treated with digital chairside CAD/CAM to make Onlay repair.The success rate of repair,the levels of inflammatory factors in gingival crevicular fluid[nuclear factor κB receptor activating factor(RANK),macrophage inflammatory protein-1α(MIP-1α)],the pain degree[numerical rating scale(NRS)]score,the periodontal indexes[gingival index(GI),gingival crevicular bleeding index(SBI),tooth mobility(TM),plaque index(PLI)]levels,the masticatory efficiency,the bite force level,and the incidence of complications.Results:The success rate of repair in the study group was 93.02% (40/43),which was higher than 75.61% (31/41)in the control group,and the difference was statistically significant(P<0.05).The chewing efficiency and bite force were higher than those of the control group,The incidence of complications and gingival crevicular fluid RANK,MIP-1α,NRS,SBI,TM,GI,and PLI scores in the study group were lower than those of the control group,and the differences were statistically significant(P<0.05).Conclusions:For dental defects,digital chair side CAD/CAM can improve the success rate of restoration,improve chewing and periodontal function,alleviate pain and inflammatory stimulation,reduce the risk of complications,and achieve better results than conventional impression based high inlay restoration.
