血清ESM-1、sICAM-1水平对帕金森病患者发生认知障碍的预测价值

目的:分析血清内皮细胞特异性分子-1(ESM-1)、可溶性细胞间黏附分子-1(sICAM-1)水平对帕金森病(PD)患者发生认知障碍的预测价值.方法:选取 2020 年 8 月至 2022 年 12 月该院收治的 93 例PD患者进行前瞻性研究,根据蒙特利尔认知评估量表(MoCA)评分将其分为认知正常组(n=50)和认知障碍组(n=43),另选取同期 50 名健康体检者,设为对照组.比较三组ESM-1、sICAM-1 水平及MoCA评分,采用Pearson相关性分析血清ESM-1、sICAM-1 水平与认知障碍的相关性,并绘制受试者工作特征(ROC)曲线分析血清ESM-1、sICAM-1 单项及联合检测预测PD患者发生认知障碍的价值.结果:认知障碍组、认知正常组血清ESM-1 水平、MoCA评分均低于对照组,且认知障碍组低于认知正常组,差异有统计学意义(P<0.05);认知障碍组、认知正常组血清sICAM-1 水平均高于对照组,且认知障碍组高于认知正常组,差异有统计学意义(P<0.05);Pearson相关性分析结果显示,血清ESM-1 水平与认知功能呈正相关(r>0,P<0.05);血清sICAM-1 水平与认知功能呈负相关(r<0,P<0.05);ROC曲线结果显示,血清ESM-1、sICAM-1 水平单项及联合预测PD患者发生认知障碍的曲线下面积分别为 0.643、0.605、0.911,其中联合检测预测价值最高.结论:血清ESM-1 水平与认知功能呈正相关,血清sICAM-1 水平与认知功能呈负相关,且血清ESM-1、sICAM-1 水平联合检测预测PD患者发生认知障碍的价值高于两者单项检测预测价值.
Correlations and predictive value of serum ESM-1 and sICAM-1 levels with cognitive impairment in patients with Parkinson's disease
Objective:To analyze correlations and predictive value of serum endothelial cell specific molecule-1(ESM-1)and soluble intercellular adhesion molecule-1(sICAM-1)levels with cognitive impairment in patients with Parkinson's disease(PD).Methods:A prospective study was conducted on 93 PD patients admitted to the hospital from August 2020 to December 2022.According to the Montreal cognitive assessment scale(MoCA)score,they were divided into normal cognitive group(n=50)and cognitive impairment group(n=43).In addition,50 healthy subjects were selected as the control group.The levels of ESM-1,sICAM-1 and MoCA scores were compared among the three groups.Pearson correlation analysis was used to analyze the correlations among the serum ESM-1,sICAM-1 levels and cognitive impairment.The receiver operating characteristic(ROC)curve was drawn to analyze the value of single and combined detection of serum ESM-1 and sICAM-1 in the prediction of cognitive impairment in the PD patients.Results:The serum ESM-1 level and the MoCA score of the cognitive impairment group and the cognitive normal group were lower than those of the control group,those of the cognitive impairment group were lower than those of the cognitive normal group,and the differences were statistically significant(P<0.05).The serum sICAM-1 levels in the cognitive impairment group and the cognitive normal group were higher than those in the control group,that in the cognitive impairment group was higher than that in the cognitive normal group,and the differences were statistically significant(P<0.05).Pearson correlation analysis showed that serum ESM-1 level was positively correlated with cognitive function(r>0,P<0.05),and the serum sICAM-1 level was negatively correlated with cognitive function(r<0,P<0.05).ROC curve showed that the area under the curve of single and combined detection of serum ESM-1 and sICAM-1 levels in the prediction of cognitive impairment in the PD patients were 0.643,0.605 and 0.911,separately,among which the combined detection had the highest predictive value.Conclusions:The serum ESM-1 level is positively correlated with cognitive function,and the serum sICAM-1 level is negatively correlated with cognitive function.Further,the value of combined detection of serum ESM-1 and sICAM-1 levels in the prediction of cognitive impairment in the PD patients is higher than that of single detection.
