Application effects of OTO model-based community health management on patients with type 2 diabetes mellitus
Objective:To explore application effects of online-to-offiine(OTO)model-based community health management on in patients with type 2 diabetes mellitus.Methods:A prospective study was conducted on 120 patients with type 2 diabetes mellitus admitted to Jingang street community health service center from March 2021 to April 2023.They were divided into control group and observation group according to the random number table method,60 cases in each group.The control group adopted routine community health management,while the observation group adopted the OTO model-based community health management.The levels of blood glucose indexes[fasting blood glucose(FBG),2 h postprandial blood glucose(2hPG),glycosylated hemoglobin(HbA1c)],the adverse psychology[self-rating depression scale(SDS),self-rating anxiety scale(SAS)]scores,and the summary of diabetes self-care activities(SDSCA)score were compared between the two groups before and after the intervention.Results:After 3 months intervention,the levels of FBG,2hPG,HbA1c,the scores of SDS and SAS in the observation group were lower than those in the control group,the SDSCA scores in the dimensions of diet,exercise,blood glucose monitoring,foot management and medication were higher than those in the control group,and the differences were statistically significant(P<0.05).Conclusions:The OTO model-based community health management for the patients with type 2 diabetes is beneficial to reduce the levels of blood glucose indexes,improve their self-management abilities,and reduce their anxiety and depression emotions.Moreover,it is superior to the routine community health management.
