Effects of double endoscopy combined with gallbladder preserving lithotomy in treatment of patients with cholecystolithiasis
Objective:To observe effects of double endoscopy combined with gallbladder preserving lithotomy in treatment of patients with cholecystolithiasis.Methods:A prospective study was conducted on 86 patients with cholecystolithiasis admitted to this hospital from August 2020 to August 2022.They were divided into control group and observation group according to the random number table method,43 cases in each group.The control group was treated with laparoscopic cholecystectomy,while the observation group was treated with double endoscopy(laparoscopy combined with choledochoscopy)combined with gallbladder preserving lithotomy.The perioperative index levels,the inflammatory factors[procalcitonin(PCT),tumor necrosis factor-α(TNF-α),interleukin-6(IL-6)]levels,the liver function index[alanine aminotransferase(ALT),aspartate aminotransferase(AST)]levels,the quality of life[functional assessment of cancer therapy-gallstones(FACT-G)]scores,and the incidence of complications were compared between the two groups.Results:The intestinal function recovery time and the hospitalization time of the observation group were shorter than those of the control group,the operation time was longer than that of control group,the intraoperative blood loss was less than that of the control group,and the differences were statistically significant(P<0.05).The levels of TNF-α,IL-6 and PCT in the two groups were lower than those before the treatment,those in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).The levels of ALT and AST in the two groups were higher than those before the treatment,but those in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).The FACT-G scores of the two groups were higher than those before the treatment,that in the observation group was higher than that in the control group,and the differences were statistically significant(P<0.05).However,there was no significant difference in the incidence of complications between the two groups(P>0.05).Conclusions:Laparoscopy combined with choledochoscopy in the treatment of the patients with cholecystolithiasis can shorten the operation time and the hospitalization time,promote the intestinal function recovery,reduce the amount of intraoperative blood loss,improve the liver function,and reduce the body's inflammatory response.Moreover,it is superior to laparoscopic cholecystectomy.
