Comparison of effects of artificial hip replacement and proximal femoral intramedullary nail internal fixation in treatment of elderly patients with intertrochanteric femoral fractures
Objective:To compare effects of artificial hip replacement(THA)and proximal femoral intramedullary nail internal fixation(PFNA)in treatment of elderly patients with intertrochanteric femoral fractures(IFF).Methods:A prospective study was conducted on 60 elderly patients with IFF admitted to this hospital from January 2021 to January 2023.According to the random number table method,they were divided into control group and observation group,30 cases in each group.The control group was treated with PFNA,while the observation group was treated with THA.The clinical efficacy,the surgical related index levels,the pain degree[visual analogue scale(VAS)]scores before and after surgery,the hip function(Harris score),and the incidence of complications were compared between the two groups.Results:The excellent and good rate of treatment in the observation group was 90.00%(27/30),which was higher than 60.00%(18/30)in the control group,and the difference was statistically significant(P<0.05).The operation time,the postoperative bed time and the start time of weight-bearing walk time in the observation group were shorter than those in the control group,the intraoperative blood loss and the postoperative drainage volume were greater than those in the control group,and the differences were statistically significant(P<0.05).Seven days after the surgery,the VAS scores of the two groups were lower than those before the surgery,that in the observation group was lower than that in the control group,and the differences were statistically significant(P<0.05).At 1,3 and 6 months after the surgery,the Harris scores of the two groups were higher than those before the surgery,those in the observation group were higher than those in the control group,and the differences were statistically significant(P<0.05).However,there was no significant difference in the incidence of complications between the two groups(P>0.05).Conclusions:THA in the treatment of the elderly patients with IFF can improve the excellent and good rate of treatment and the Harris scores,improve the levels of operation-related indicators,and reduce the VAS scores.It is superior to PFNA treatment,but the intraoperative blood loss is more than that of PFNA treatment.
