Comparison of effects of laparotomy myomectomy and laparoscopic myomectomy in treatment of patients with uterine fibroids
Objective:To compare effects of laparotomy myomectomy and laparoscopic myomectomy in treatment of patients with uterine fibroids.Methods:A prospective study was conducted on 140 patients with uterine fibroids admitted to the hospital from January 2019 to January 2020.According to the random number table method,they were divided into observation group and control group,70 cases in each group.The control group was treated with laparotomy myomectomy,while the observation group was treated with laparoscopic myomectomy.The levels of clinical related indexes(intraoperative blood loss,operation time,hospitalization time,postoperative first exhaust time),the pain visual analogue scale(VAS)scores at different time after the surgery(1 and 3 days after the surgery),the serum oxidative stress indexes[malondialdehyde(MDA),total antioxidant capacity(T-AOC),glutathione peroxidase(GSH-Px)]levels,and the incidence of complications were compared between the two groups.Results:The intraoperative blood loss in the observation group was less than that in the control group,the operation time,hospitalization time and the postoperative first exhaust time were shorter than those in the control group,and the differences were statistically significant(P<0.05).1 and 3 days after the surgery,the VAS score of the observation group was lower than that of the control group,and the difference was statistically significant(P<0.05).At 1 and 3 days after the surgery,the serum malondialdehyde levels in the two groups were higher than those before the surgery,but those in the observation group were lower than those in the control group;the levels of serum T-AOC and GSH-Px in the two groups were lower than those before the surgery,but those in the observation group were higher than those in the control group;and the differences were statistically significant(P<0.05).Further,the incidence of complications in the observation group was 1.43%,which was lower than 11.43%in the control group,and the difference was statistically significant(P<0.05).Conclusions:Laparoscopic myomectomy for the patients with uterine fibroids can reduce the intraoperative blood loss,the postoperative VAS scores,the incidence of complications,shorten the operation time,the hospitalization time,the postoperative first exhaust time,and improve the levels of oxidative stress indexes.Moreover,it is superior to laparotomy myomectomy.
