Effects of Nalmefene in adjuvant treatment of elderly patients with acute exacerbation of chronic obstructive pulmonary disease complicated with type Ⅱ respiratory failure
Objective:To investigate effects of Nalmefene in adjuvant treatment of elderly patients with acute exacerbation of chronic obstructive pulmonary disease(AECOPD)complicated with type II respiratory failure.Methods:A prospective study was conducted on 116 elderly patients with AECOPD and type II respiratory failure admitted to the hospital from May 2020 to April 2023.According to the random number table method,they were divided into control group and study group,58 cases in each group.Both groups were given nasal low-flow oxygen therapy.On this basis,the control group was treated with Salmeterol/Fluticasone combined with Ambroxol,while the study group was treated with Nalmefene hydrochloride injection on the basis of those of the control group.The clinical efficacy,the lung function indexes[forced expiratory volume in one second(FEV1),FEV1/forced vital capacity(FVC)]levels,the exercise tolerance[6 min walking test distance(6MWD)],the blood gas indexes[arterial partial pressure of carbon dioxide(PaCO2),arterial oxygen saturation(SaO2)]levels,the serum inflammatory factors[leukotriene B4(LTB4),chemokine receptor 4(CXCR4)]levels,and the incidence of adverse reactions were compared between the two groups before and after the treatment.Results:The total effective rate of the observation group was 89.66%(52/58),which was higher than 75.86%(44/58)of the control group,and the difference was statistically significant(P<0.05).After the treatment,the levels of FEV1,FEV1/FVC,SaO2 and 6MWD in the observation group were higher than those in the control group,the levels of PaCO2,CXCR4 and LTB4 were lower than those in the control group,and the differences were statistically significant(P<0.05).However,there was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusions:Nalmefene combined with conventional therapy in the treatment of the elderly AECOPD patients complicated with type II respiratory failure can accelerate the relief of symptoms,improve the lung function and the exercise tolerance,improve the blood gas index levels,and reduce the levels of inflammatory factors.Moreover,it is superior to single conventional therapy.
