目的:分析人附睾蛋白 4(HE4)、癌胚抗原 125(CA125)、糖类抗原 15-3(CA15-3)水平联合检测在卵巢肿瘤良恶性诊断中的效能.方法:选取 2022 年 1 月至 2023 年 6 月该院收治的 78 例卵巢肿瘤患者进行横断面研究,依据病理学检查结果将其分为卵巢癌组 48 例和卵巢良性病变组 30 例,另选取同期 30 名体检的健康女性设为对照组.比较三组、不同分期卵巢癌组患者血清HE4、CA125、CA15-3 水平,绘制受试者工作特征(ROC)曲线分析HE4、CA125、CA15-3 水平单项及联合检测在卵巢肿瘤良恶性诊断中的效能.结果:卵巢癌组HE4、CA125、CA15-3 水平均高于卵巢良性病变组、对照组,卵巢良性病变组CA125 水平高于对照组,差异有统计学意义(P<0.05);卵巢良性病变组与对照组HE4、CA15-3 水平比较,差异均无统计学意义(P>0.05);Ⅲ~Ⅳ期卵巢癌患者HE4、CA125、CA15-3 水平均高于Ⅰ~Ⅱ期患者,差异有统计学意义(P<0.05);ROC曲线分析结果显示,HE4、CA125、CA15-3 水平单项及联合检测诊断卵巢癌的曲线下面积分别为 0.731、0.695、0.636、0.953,且联合检测诊断效能高于三者单项检测诊断效能.结论:血清HE4、CA125、CA15-3 水平联合检测诊断卵巢肿瘤良恶性的效能高于三者单项检测诊断.
Efficiency of combined detection of serum HE4,CA125 and CA15-3 levels in diagnosis of benign and malignant ovarian tumors
Objective:To analyze efficiency of combined detection of human epididymis protein 4(HE4),carcinoembryonic antigen 125(CA125)and carbohydrate antigen 15-3(CA15-3)in diagnosis of benign and malignant ovarian tumors.Methods:A cross-sectional study was conducted on 78 patients with ovarian tumors admitted to this hospital from January 2022 to June 2023.According to the results of pathological examination,they were divided into ovarian cancer group(48 cases)and ovarian benign lesion group(30 cases).Another 30 healthy women who underwent physical examination during the same period were selected as the control group.The levels of serum HE4,CA125 and CA153 were compared among the three groups and the patients with different stages of ovarian cancer.The receiver operating characteristic(ROC)curve was drawn to analyze the efficiencies of single and combined detection of HE4,CA125 and CA15-3 levels in the diagnosis of benign and malignant ovarian tumors.Results:The levels of HE4,CA125 and CA15-3 in the ovarian cancer group were higher than those in the ovarian benign lesion group and the control group,the level of CA125 in the ovarian benign lesion group was higher than that in the control group,and the differences were statistically significant(P<0.05).There was no statistically significant differences in the HE4 and CA15-3 levels between the ovarian benign lesions group and the control group(P>0.05).The levels of HE4,CA125 and CA15-3 in the patients with ovarian cancer stage Ⅲ-Ⅳ were higher than those in the patients with stage Ⅰ~Ⅱ,and the differences were statistically significant(P<0.05).The ROC curve analysis showed that the area under the curve of HE4,CA125 and CA15-3 levels in the diagnosis of malignant ovarian tumors were 0.731,0.695,0.636 and 0.953,respectively,and the diagnostic efficiency of combined detection was higher than that of single detection.Conclusions:The efficiency of combined detection of serum HE4,CA125 and CA15-3 levels in the diagnosis of benign and malignant ovarian tumors is higher than that of single detection of the three.
