differential diagnosis efficiency of contrast-enhanced ultrasound combined with ultrasound elastography in distinguishing benign and malignant breast nodules with clinical palpation negative
Objective:To explore differential diagnostic efficiency of contrast-enhanced ultrasound combined with ultrasound elastography in distinguishing benign and malignant breast nodules with clinical palpation negative. Methods:The clinical data of 120 patients with clinical palpation negative breast nodules who were treated in the hospital from June 2020 to June 2022 were retrospectively analyzed. The contrast-enhanced ultrasound parameters[peak intensity(PI),wash in slope(WIS),area under the curve(AUC),time to peak(TTP)]levels, the ultrasound elastography parameters[area percentage of blue area(AREA%),mean strain of interest(MEAN)and standard deviation(SD)]of benign and malignant nodules were compared.Using the pathological results as the gold standard, the efficiencies of contrast-enhanced ultrasound, ultrasound elastography single and combination examinations for the differential diagnosis of benign and malignant breast nodules with clinical palpation negative. Results:The pathological examination showed that there were 74 cases(61.67%)of malignant nodules and 46 cases(38.33%)of benign nodules in the 120 patients with palpation negative breast nodules. The levels of PI,WIS,AUC,AREA%,MEAN and SD of the malignant nodules were higher than those of the benign nodules, the TTP value was lower than that of the benign nodules, and the differences were statistically significant(P<0.05).The sensitivity and the accuracy of combined examination of contrast-enhanced ultrasound and ultrasound elastography in diagnosing clinical palpable negative breast malignant nodules were higher than those of single diagnosis, and the differences were statistically significant(P<0.05).There was no significant difference in the specificity of single and combined examination of contrast-enhanced ultrasound and ultrasound elastography in diagnosing clinical palpable negative breast malignant nodules(P>0.05).Conclusions:Contrast-enhanced ultrasound and ultrasonic elastography have certain diagnostic value for clinical palpation negative breast nodules.Further, the sensitivity and the accuracy of combined diagnosis are higher than those of single contrast-enhanced ultrasound and singe ultrasound elastography.
