齿痛消炎灵颗粒联合甲硝唑对慢性根尖周炎患者根管治疗后炎症和疼痛的影响
Effects of Chitong Xiaoyanling granules combined with Metronidazole on inflammation and pain after root canal therapy in patients with chronic apical periodontitis

摘要
目的:探讨齿痛消炎灵颗粒联合甲硝唑对慢性根尖周炎患者根管治疗后炎症和疼痛的影响.方法:选取 2021 年 10 月至2023年10月在该院行根管治疗的95例慢性根尖周炎患者进行前瞻性研究,按照随机数字表法将其分为对照组(n=47)和观察组(n=48).在根管治疗基础上,对照组口服甲硝唑片治疗,观察组在对照组基础上联合齿痛消炎灵颗粒治疗.比较两组临床疗效,治疗前后牙周指标[牙龈指数(GI)、菌斑指数(PLI)]、龈沟液炎性因子[肿瘤坏死因子-α(TNF-α)、白细胞介素-1β(IL-1β)、白细胞介素-6(IL-6)]水平、视觉模拟评分法(VAS)评分,以及不良反应发生率.结果:观察组治疗总有效率为 91.67%(44/48),高于对照组的76.60%(36/47),差异有统计学意义(P<0.05);治疗 7 d后,观察组GI、PLI、VAS评分及龈沟液TNF-α、IL-1β、IL-6 水平均低于对照组,差异有统计学意义(P<0.05);两组不良反应发生率比较,差异无统计学意义(P>0.05).结论:齿痛消炎灵颗粒联合甲硝唑用于慢性根尖周炎根管治疗患者可提高临床疗效,改善牙周指标,降低龈沟液炎性因子水平,减轻牙齿疼痛,效果优于单用甲硝唑治疗.
Abstract
Objective:To investigate effects of Chitong Xiaoyanling granules combined with Metronidazole on inflammation and pain after root canal therapy in patients with chronic apical periodontitis.Methods:A total of 95 patients with chronic apical periodontitis who underwent root canal treatment in this hospital from October 2021 to October 2023 were selected.According to the random number table method,they were divided into control group(n=47)and observation group(n=48).On the basis of the root canal treatment,the control group was treated with Metronidazole tablets,while the observation group was treated with Chitong Xiaoyanling granules on the basis of that of the control group.The clinical efficacy,the periodontal indexes[gingival index(GI),plaque index(PLI)]levels,the gingival crevicular fluid inflammatory factors[tumor necrosis factor-α(TNF-α),interleukin-1β(IL-1β),interleukin-6(IL-6)]levels,visual analogue scale(VAS)score before and after the treatment,and the incidence of adverse reactions were compared between the two groups.Results:The total effective rate of the observation group was 91.67%(44/48),which was higher than 76.60%(36/47)of the control group,and the difference was statistically significant(P<0.05).After 7 days of treatment,the scores of GI,PLI,VAS and the levels of TNF-α,IL-1β and IL-6 in gingival crevicular fluid in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).However,there was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusions:Chitong Xiaoyanling granules combined with Metronidazole can improve the clinical efficacy,improve the periodontal index levels,reduce the levels of inflammatory factors in the gingival crevicular fluid and relieve the tooth pain in the patients with chronic apical periodontitis after root canal treatment.Moreover,it is superior to single Metronidazole treatment.
