Efficiency of multi-spiral CT angiography in diagnosis of carotid artery stenosis
Objective:To observe efficiency of multi-spiral CT angiography(MSCTA)in diagnosis of carotid artery stenosis(CAS).Methods:A cross-sectional study was conducted on 112 patients with suspected CAS admitted to the hospital from June 2020 to May 2022.All patients were given MSCTA examination and color Doppler ultrasound(CDUS)examination.Using the results of digital subtraction angiography(DSA)as the'gold standard',the efficiency of MSCTA and CDUS in the diagnosis of CAS,and the detection rates of CAS with different degrees of stenosis were compared.Results:DSA showed that 87 of 112 patients with suspected CAS were positive,including 18 cases of mild stenosis,17 cases of moderate stenosis,38 cases of severe stenosis and 14 cases of occlusion.There were 25 cases were negative,including 15 cases of cerebral infarction and 10 cases of carotid aneurysm.MSCTA showed that 87 cases were positive and 25 cases were negative.CDUS examination showed that 82 cases were positive and 30 cases were negative.There were no significant differences in the specificity,the misdiagnosis rate and the positive predictive value between MSCTA and CDUS(P>0.05).The sensitivity,the accuracy and the negative predictive value of MSCTA in the diagnosis of CAS were higher than those of CDUS,and the differences were statistically significant(P<0.05).There was no statistically significant difference in the detection rate of mild to moderate stenosis CAS between MSCTA examination and CTA examination,the difference was not statistically significant(P>0.05).Further,the detection rate of CAS in the diagnosis of severe stenosis or occlusion by MSCTA was higher than that by CDUS,and the difference was statistically significant(P<0.05).Conclusions:The diagnostic efficiency of MSCTA in the diagnosis of CAS is higher than that of CDUS.
