Comparison of effects of transforaminal lumbar interbody fusion and posterior lumbar interbody fusion with bone graft in treatment of elderly patients with degenerative lumbar spondylolisthesis
Objective:To compare effects of transforaminal lumbar interbody fusion(TLIF)and posterior lumbar interbody fusion(PLIF)with bone graft in treatment of elderly patients with degenerative lumbar spondylolisthesis.Methods:The clinical data of 110 elderly patients with degenerative lumbar spondylolisthesis admitted to this hospital from July 2020 to July 2022 were retrospectively analyzed.According to different surgical approaches,they were divided into study group(53 cases)and control group(57 cases).The study group was treated with TLIF,while the control group was treated with PLIF.The perioperative indexes(operation time,intraoperative blood loss,postoperative drainage volume and hospitalization time),the pain degree[visual analogue scale(VAS)]score before and after the surgery,the lumbar function[Japanese Orthopaedic Association(JOA)score for low back pain,the Oswestry dysfunction index(ODI)]score,and the incidence of postoperative complications were compared between the two groups.Results:The operation time and the hospitalization time of the study group were shorter than those of the control group,the intraoperative blood loss and postoperative drainage volume were both lower than those of the control group,and the differences were statistically significant(P<0.05).At 6 and 12 months after the surgery,the VAS scores of both groups were lower than those before the surgery,but there were no statistically significant differences between the groups(P>0.05).At 12 months after the surgery,the JOA scores of low back pain in the two groups were higher than those before the surgery,and the ODI scores were lower than were before the surgery,but there were no significant differences between the two groups(P>0.05).There were no statistically significant differences in the incidence of incision infection,lung infection,and healthy side nerve injury between the two groups(P>0.05).The incidence of nerve injury on the affected side in the study group was lower than that in the control group,and the difference was statistically significant(P<0.05).Conclusions:TLIF and PLIF in the treatment of the elderly patients with degenerative lumbar spondylolisthesis can effectively reduce the degree of pain and restore the lumbar function,but TLIF can improve the levels of perioperative indexes and reduce the incidence of lateral nerve injury.Moreover,it is superior to PLIF treatment.
