目的:观察沙丁胺醇联合无创机械通气治疗慢性阻塞性肺疾病急性加重期(AECOPD)患者的效果.方法:回顾性分析2022 年 1 月至 2023 年 3 月该院收治的 92 例AECOPD患者的临床资料,按照治疗方法不同将其分为对照组和研究组各 46 例.两组均予以常规治疗,在此基础上,对照组予以无创机械通气治疗,研究组在对照组基础上联合沙丁胺醇治疗.比较两组临床疗效、治疗前后肺功能指标[用力肺活量(FVC)、第 1 秒用力呼气容积(FEV1)、FEV1/FVC]水平、氧代谢指标[动脉血氧分压(PaO2)、血乳酸(Lac)、氧耗指数(DO2)、氧摄取率(O2ER)]水平、炎性指标[白细胞介素(IL)-13、IL-17、IL-35、可溶性髓样细胞触发受体-1(sTREM-1)]水平以及不良反应/并发症发生率.结果:研究组治疗总有效率为 93.48%(43/46),高于对照组的 78.26%(36/46),差异有统计学意义(P<0.05);治疗后,研究组FEV1、FVC、FEV1/FVC水平均高于对照组,差异有统计学意义(P<0.05);治疗后,研究组PaO2、O2ER、DO2 水平高于对照组,Lac水平低于对照组,差异均有统计学意义(P<0.05);治疗后,研究组IL-13、IL-17、sTREM-1 水平低于对照组,IL-35 水平高于对照组,差异均有统计学意义(P<0.05);两组不良反应/并发症发生率比较,差异无统计学意义(P>0.05).结论:沙丁胺醇联合无创机械通气治疗AECOPD患者可提高治疗总有效率、肺功能指标水平,改善氧代谢指标、炎性指标水平,效果优于单纯无创机械通气治疗.
Effects of Salbutamol combined with noninvasive mechanical ventilation in treatment of patients with acute exacerbation of chronic obstructive pulmonary disease
Objective:To observe effects of Salbutamol combined with noninvasive mechanical ventilation in treatment of patients with acute exacerbation of chronic obstructive pulmonary disease(AECOPD).Methods:The clinical data of 92 patients with AECOPD admitted to this hospital from January 2022 to March 2023 were retrospectively analyzed.According to different treatment methods,they were divided into control group and study group,46 cases in each group.Both groups were given routine treatment.On this basis,the control group was treated with noninvasive mechanical ventilation,while the study group was treated with Salbutamol on the basis of that of the control group.The clinical efficacy,the levels of lung function indexes[forced vital capacity(FVC),forced expiratory volume in one second(FEV1),FEV1/FVC],oxygen metabolism indexes[arterial partial pressure of oxygen(PaO2),blood lactic acid(Lac),oxygen consumption index(DO2),oxygen uptake rate(O2ER)]and inflammatory indexes[interleukin(IL)-13,IL-17,IL-35,soluble myeloid cell trigger receptor-1(sTREM-1)],and the incidence of adverse reactions or complications were compared between the two groups.Results:The total effective rate of the study group was 93.48%(43/46),which was higher than 78.26%(36/46)of the control group,and the difference was statistically significant(P<0.05).After the treatment,the levels of FEV1,FVC and FEV1/FVC in the study group were higher than those in the control group,and the differences were statistically significant(P<0.05).After the treatment,the levels of PaO2,O2ER and DO2 in the study group were higher than those in the control group,the level of Lac was lower than that in the control group,and the differences were statistically significant(P<0.05).After the treatment,the levels of IL-13,IL-17 and sTREM-1 in the study group were lower than those in the control group,the level of IL-35 was higher than that in the control group,and the differences were statistically significant(P<0.05).However,there was no significant difference in the incidence of adverse reactions or complications between the two groups(P>0.05).Conclusions:Salbutamol combined with noninvasive mechanical ventilation in the treatment of the AECOPD patients can increase the total effective rate and the lung function index levels,improve the oxygen metabolism index and inflammatory index levels.Moreover,it is superior to single noninvasive mechanical ventilation treatment.
