Efficiency of color Doppler ultrasonography in diagnosis of renal artery stenosis
Objective:To observe efficiency of color Doppler ultrasonography in diagnosis of renal artery stenosis. Methods:A cross-sectional study was conducted on 67 patients with suspected renal artery stenosis admitted to the hospital from August 2022 to August 2023.All patients were given color Doppler ultrasonography and renal angiography.Using the results of renal angiography as the"gold standard",the diagnostic results and the diagnostic efficiency of color Doppler ultrasonography were analyzed. At the same time, the consistency between color Doppler ultrasonography and renal angiography in the diagnosis of the patients with different degrees of renal artery stenosis was analyzed. The levels of color Doppler ultrasonography parameters[peak systolic velocity(PSV),interlobar artery resistance index(RI),end-diastolic peak velocity(EDV),ratio of renal artery peak flow/abdominal aorta peak flow(RAR)]in the patients with different degrees of renal artery stenosis were compared. Results:Suspected renal angiography showed that 54 of the 67 patients with renal artery stenosis were positive, including 20 cases of mild stenosis,18 cases of moderate stenosis,10 cases of severe stenosis and 6 cases of complete occlusion, and 13 cases were negative. The color Doppler ultrasonography showed that 53 cases were positive and 14 cases were negative. The sensitivity, the accuracy and the specificity of color Doppler ultrasonography were 96.30%,95.52%and 92.31%.The consistency with the gold standard diagnosis was good(Kappa=0.869).The consistency between color Doppler ultrasonography and renal angiography in the diagnosis of the patients with different degrees of stenosis was good(Kappa value=0.895).The levels of EDV,RAR and PSV in the patients with different degrees of renal artery stenosis were mild<moderate<severe or occlusion;the RI level was mild>moderate>severe or occlusion, and the differences were statistically significant(P<0.05).Conclusions:The efficiency of color Doppler ultrasonography in the diagnosis of renal artery stenosis is in good agreement with that of renal angiography.
