Application effects of color Doppler ultrasound combined with CT angiography in preoperative localization of lower extremity perforator vessels
Objective:To analyze application effects of color Doppler ultrasound(CDU)combined with CT angiography(CTA)in preoperative localization of lower extremity perforator vessels. Methods:The clinical data of 200 patients with lower extremity soft tissue defect repair admitted to this hospital from June 2022 to June 2023 were retrospectively analyzed.According to different vascular localization methods, they were divided into CDU group(n=66),CTA group(n=66)and combined group(n=68).The recognition rate of dominant perforator vessels, the accuracy rate of dominant perforator vessels localization, the examination time, the flap removal time, the success rate of flap removal, the incidence of complications, and the postoperative quality of life[Barthel index(BI)]score were compared among the three groups. Results:There was no significant difference in the recognition rate of dominant perforator vessels among the three groups(P>0.05).The accuracy rate of dominant perforator vessels localization in the combined group was higher than that in the CDU group, the difference distance was lower than that in the CDU group and the CTA group, and the differences were statistically significant(P<0.05).There were no statistical differences in the accuracy rate of dominant perforator vessels localization and the difference distance between the CDU group and the CTA group. There was no statistical difference in the accuracy rate of dominant perforator vessels localization between the combined group and the CTA group. The examination time of the combined group was longer than that of the CDU group and the CTA group, the flap removal time was shorter than that of the CDU group and the CTA group, the success rate of flap removal was higher than that of the CDU group and the CTA group, and the differences were statistically significant(P>0.05).There were no significant differences in the examination time, the flap removal time and the success rate of flap removal between the CDU group and the CTA group(P>0.05).The BI score of the combined group was higher than that of the CDU group and the CTA group, the incidence of complications was lower than that of the CDU group and the CTA group, and the differences were statistically significant(P>0.05).There was no significant differences in the BI score and the incidence of complications between CDU group and CTA group(P>0.05).Conclusions:CDU combined with CTA in the preoperative positioning of lower extremity perforator vessels can improve the accuracy rate of dominant perforator vessels localization, the success rate of flap removal and the quality of life scores, shorten the time and distance of flap removal, and reduce the incidence of complications. Moreover,it is superior to two single examinations, but the examination time is longer.
