Comparison of effects of proximal femoral intramedullary nail internal fixation and hip replacement in treatment of patients with hip fracture
Objective:To compare effects of proximal femoral intramedullary nail internal fixation and hip replacement in treatment of patients with hip fracture.Methods:The clinical data of 90 patients with hip fracture admitted to this hospital from January 2021 to July 2023 were retrospectively analyzed.They were divided into observation group and control group according to different surgical methods,45 cases in each group.The control group was treated with hip replacement,while the observation group was treated with proximal femoral intramedullary nail internal fixation.The perioperative indexes(operation time,intraoperative blood loss,fracture healing time,postoperative hospitalization time)levels,the pain[visual analogue scale(VAS)]score,the hip joint function[Harris hip scale(HHS)]score,the bone metabolism index[osteocalcin(BGP),osteoprotegerin(OPG),bone mineral density(BMD)]levels,and the incidence of complications were compared between the two groups before and after the surgery.Results:The operation time of the observation group was shorter than that of the control group,the intraoperative blood loss was less than that of the control group,and the differences were statistically significant(P<0.05).3 and 7 days after the surgery,the VAS scores of the observation group were lower than those of the control group,and the differences were statistically significant(P<0.05).1 and 3 months after the surgery,the HSS scores of the observation group were higher than those of the control group,and the differences were statistically significant(P<0.05).3 months after the surgery,the levels of BGP,OPG and BMD in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).However,there were no significant differences in fracture healing time,postoperative hospitalization time and complication rate between the two groups(P>0.05).Conclusions:Proximal femoral intramedullary nail internal fixation in the treatment of the patients with hip fracture can shorten the operation time,reduce the intraoperative blood loss,reduce the postoperative pain scores,and improve the postoperative hip function scores.It is superior to hip replacement,but the improvement of bone metabolism index is not as good as that of hip replacement.
