Application effects of continuous nursing based on IMPACT model in elderly patients with type 2 diabetes mellitus complicated with coronary heart disease
Objective:To observe application effects of continuous nursing based on IMPACT model in elderly patients with type 2 diabetes mellitus complicated with coronary heart disease.Methods:A prospective study was conducted on 94 elderly patients with type 2 diabetes mellitus complicated with coronary heart disease admitted to the hospital from August 2020 to August 2022.They were divided into observation group and control group by using the random number table method,47 cases in each group.The control group received routine nursing,while the observation group received the continuous nursing based on IMPACT model.The levels of blood glucose indexes[2 h postprandial blood glucose(2hPG),fasting blood glucose(FBG),glycosylated hemoglobin(HbA1c)],cardiac function indexes[left ventricular ejection fraction(LVEF),left ventricular short axis shortening rate(FS),left ventricular end-systolic diameter(LVESD),left ventricular end-diastolic diameter(LVEDD)],health behavior[health-promoting lifestyle profile Ⅱ(HPLP-Ⅱ)]score,quality of life[type 2 diabetes mellitus quality of life scale(DMQLS)]score,and the nursing satisfaction were compared between the two groups before and after the nursing.Results:After the nursing,the levels of 2hPG,FBG and HbA1c in the two groups were lower than those before the nursing,those in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).The levels of LVEF and FS in the two groups were higher than those before the nursing,and those in the observation group were higher than those in the control group;the levels of LVEDD and LVESD in the two groups were lower than those before the nursing,and those in the observation group were lower than those in the control group;and the differences were statistically significant(P<0.05).The HPLP-Ⅱ scores of stress management,health responsibility,interpersonal relationship,nutrition,sports and mental growth in the two groups were higher than those before the nursing,those in the observation group were higher than those in the control group,and the differences were statistically significant(P<0.05).The DMQLS scores of disease,physiological,psychological,social and satisfaction in the two groups were higher than those before the nursing,those in the observation group were higher than those in the control group,and the differences were statistically significant(P<0.05).Further,the nursing satisfaction of the observation group was 95.74%(45/47),which was higher than 82.98%(39/47)of the control group,and the difference was statistically significant(P<0.05).Conclusions:The continuous nursing based on IMPACT model in the elderly patients with type 2 diabetes mellitus complicated with coronary heart disease can reduce the levels of blood glucose indexes,improve the levels of cardiac function indexes,and improve the health behavior scores,quality of life scores and nursing satisfaction.Moreover,it is superior to the routine nursing.
