Analysis of pathogen distribution and drug resistance in 215 elderly patients with T2DM complicated with urinary tract infection
Objective:To analyze distribution and drug resistance of pathogenic bacteria in 215 elderly patients with type 2 diabetes mellitus(T2DM)complicated with urinary tract infection.Methods:The clinical data of 215 elderly patients with T2DM complicated with urinary tract infection admitted to this hospital from March 2021 to March 2023 were retrospectively analyzed.The urine samples were collected for pathogen culture and drug sensitivity test.The distribution of pathogenic bacteria in these elderly patients with T2DM complicated with urinary tract infection was statistically analyzed.The drug resistance of main Gram-negative bacteria and Gram-positive bacteria to antibiotics was analyzed.Results:221 strains of pathogenic bacteria were isolated from the urine samples of the 215 elderly patients with T2DM complicated with urinary tract infection,including 177 strains of Gram-negative bacteria,accounting for the highest proportion(80.10%),mainly Escherichia coli,Klebsiella pneumoniae,Proteus mirabilis;there were 38 strains of Gram-positive bacteria,accounting for 17.19%,mainly Enterococcus faecalis;and there were 6 strains of fungi,accounting for 2.71%.Escherichia coli had the highest resistance to Ampicillin(56.91%),followed by Piperacillin(53.66%)and Etimicin(51.22%);the resistance to Amikacin and Tazobactam was low(<10%);and it was not resistant to Imipenem.Klebsiella pneumoniae had the highest resistance to Ciprofloxacin and Ampicillin(both 55.00%),and was not resistant to Imipenem.The resistance rate of Enterococcus faecalis to Tetracycline was the highest(82.35%),followed by Erythromycin,Ciprofloxacin,Streptomycin(all 58.82%)and Chloramphenicol(52.94%),and it was not resistant to Penicillin G,Ampicillin and Vancomycin.Conclusions:Gram-negative bacteria are the main pathogenic bacteria in the elderly patients with T2DM complicated with urinary tract infection,among which Escherichia coli accounts for the highest proportion,and has high resistance to Ampicillin,Piperacillin and Etimicin.
