Application effects of bronchoscopic alveolar lavage combined with conventional treatment in patients with chronic obstructive pulmonary disease complicated with acute respiratory failure
Objective:To observe application effects of bronchoscopic alveolar lavage combined with conventional treatment in patients with chronic obstructive pulmonary disease(COPD)complicated with acute respiratory failure.Methods:A prospective study was conducted on 80 patients with COPD complicated with acute respiratory failure admitted to the hospital from January 2021 to December 2023.According to the random number table method,they were divided into study group and control group,40 cases in each group.The control group was treated with conventional therapy(Salmeterol Xinafoate and Fluticasone Propionate inhalation aerosol,Salbutamol inhalation aerosol inhalation therapy and noninvasive mechanical ventilation therapy),while the study group was treated with bronchoscopy alveolar lavage on the basis of that of the control group.The clinical efficacy,the pulmonary function indexes[forced expiratory volume in one second(FEV1),forced vital capacity(FVC)]levels,the inflammatory factors[C-reactive protein(CRP),interleukin-6(IL-6)and procalcitonin(PCT)]levels,and the T cell subsets indexes(CD3+,CD4+and CD8+)levels were compared between the two groups.Results:The total effective rate of the study group was 92.50%,which was significantly higher than 75.00%of the control group,and the difference was statistically significant(P<0.05).After the treatment,the levels of FEV1,FVC,CD3+and CD4+in the two groups were higher than those before the treatment,and those in the study group were higher than those in the control group;the levels of PCT,CRP,IL-6 and CD8+in the two groups were lower than those before the treatment,and those in the study group were lower than those in the control group;and the differences were statistically significant(P<0.05).Conclusions:Bronchoscopic alveolar lavage combined with conventional treatment can improve the total effective rate and the pulmonary function index levels,improve the levels of T cell subsets,and reduce the levels of inflammatory factors in the patients with COPD complicated with acute respiratory failure.Moreover,it is superior to single conventional treatment.
