Effects of endoscopic thyroidectomy via chest-breast approach in treatment of patients with benign thyroid tumors
Objective:To observe effects of endoscopic thyroidectomy via chest-breast approach in treatment of patients with benign thyroid tumors. Methods:A prospective study was conducted on 60 patients with benign thyroid tumors admitted to this hospital from January to December 2022.According to the random number table method, they were divided into observation group and control group,30 cases in each group. The control group was treated with open thyroidectomy, while the observation group was treated with endoscopic thyroidectomy via chest-breast approach. The perioperative index levels, the thyroid function indexes[free triiodothyronine(FT3),free thyroxine(FT4),thyroid stimulating hormone(TSH)]levels before and after the surgery, the pain degree[visual analogue scale(VAS)]score, the scar beauty[Vancouver scar scale(VSS)]score, the incidence of complications, and the recurrence rate were compared between the two groups. Results:The operation time of the observation group was longer than that of the control group, the intraoperative blood loss and the postoperative drainage volume were less than those of the control group, the recovery time of neck activity and the hospitalization time were shorter than those of the control group, and the differences were statistically significant(P<0.05).3 days after the surgery, the levels of FT3 and FT4 in the two groups were lower than those before the surgery, but those in the observation group were higher than those in the control group;the TSH levels of the two groups were higher than those before the surgery, but that in the observation group was lower than that in the control group;and the differences were statistically significant(P<0.05).The VAS and VSS scores of the observation group were lower than those of the control group, and the differences were statistically significant(P<0.05).The incidence of complications in the observation group was lower than that in the control group, the difference was statistically significant(P<0.05).However,there was no significant difference in the recurrence rate between the two groups(P>0.05).Conclusions:Endoscopic thyroidectomy via chest-breast approach for the patients with benign thyroid tumors can reduce the intraoperative blood loss and the postoperative drainage, shorten the neck activity recovery time and the hospitalization time, improve the thyroid function index levels, and reduce the VAS scores, the VSS scores and the incidence of complications. Moreover,it is superior to open thyroidectomy, but the operation time needs to be prolonged.
