Effects of internal fixation through posterolateral fibula approach in treatment of patients with supination external rotation type Ⅳ ankle fracture
Objective:To observe effects of internal fixation through posterolateral fibula approach in treatment of patients with supination external rotation type Ⅳ ankle fracture(AF).Methods:The clinical data of 80 patients with supination external rotation type Ⅳ AF admitted to the hospital from March 2019 to October 2023 were retrospectively analyzed.According to different surgical methods,they were divided into control group and observation group,40 cases in each group.The control group was treated with internal fixation via fibular approach,while the observation group was treated with internal fixation through posterolateral fibular approach.Both groups were followed up for 6 months.The perioperative indexes(intraoperative blood loss,operation time,fracture healing time,hospitalization time)levels,the excellent and good rate of fracture healing,the excellent and good rate of ankle joint function recovery,and the incidence of complications were compared between the two groups.Results:The intraoperative blood loss in the observation group was less than that in the control group,the fracture healing time and the hospitalization time were shorter than those in the control group,and the differences were statistically significant(P<0.05).There was no significant difference in operation time between the two groups(P>0.05).The excellent and good rate of fracture healing in the observation group was 95.00%(38/40),which was higher than 80.00%(32/40)in the control group,and the difference was statistically significant(P<0.05).The excellent and good rate of ankle joint function recovery in the observation group was 82.50%(33/40),which was higher than 62.50%(25/40)in the control group,and the difference was statistically significant(P<0.05).Further,the incidence of complications in the observation group was 5.00%(2/40),which was lower than 20.00%(8/40)in the control group,and the difference was statistically significant(P<0.05).Conclusions:Internal fixation through posterolateral fibula approach in the treatment of the patients with supination external rotation type Ⅳ AF can improve the levels of perioperative indexes,improve the excellent and good rate of fracture healing and the excellent and good rate of ankle function recovery,and reduce the incidence of complications.Moreover,it is superior to internal fixation via fibular approach.
